You probably know how quickly Medicare premiums add up each month, especially when you are on a fixed income. For many seniors, that monthly bill is one of the biggest standing charges in the household budget.
This month, a small piece of that cost is landing back in their accounts.
The federal government is sending a one-time $90 payment to more than 20 million Medicare Part B enrollees, according to a White House fact sheet released in early October 2026.
The money is designed to offset Part B premium costs and arrives automatically for eligible seniors. No application is needed.

How the payment arrives
Direct deposits of $90 began landing in early October 2026. The money is routed through the same account the Social Security Administration already uses for monthly benefits.
People without direct deposit on file receive a paper check mailed to the address Medicare has on record. The check is marked as a Medicare Improvement Fund payment.
The money comes out of the Medicare Improvement Fund, a $2 billion reserve Congress set up for Medicare program improvements. The fund has sat unused since lawmakers created it in 2008.
According to the White House, this is the first time it has been drawn on to deliver money directly to beneficiaries.

Larry Levitt, KFF's executive vice president for health policy, told STAT that the fund gives the administration unusual flexibility. "The Medicare Improvement Fund language is quite broad and gives substantial discretion to the administration," Levitt said.
Who does not qualify
Most Part B enrollees are eligible, but several large groups are left out. Seniors enrolled in Medicare Advantage, the private plan alternative to traditional Medicare, do not receive the payment.
The $90 also does not go to seniors whose Part B premiums are already paid by Medicaid. Higher earners who pay an Income-Related Monthly Adjustment Amount, known as IRMAA, are excluded as well.
The IRMAA surcharge kicks in above $109,000 in modified adjusted gross income for single filers and $218,000 for a married couple filing jointly. Enrollees over that threshold pay a higher monthly premium and are not on the $90 list.

What $90 covers in context
The standard monthly Part B premium is $202.90 in 2026, according to the Centers for Medicare and Medicaid Services. That is an increase of $17.90 from the $185.00 standard premium in 2025.
The $90 payment covers a little under half of one month's standard premium. The annual Part B deductible also rose this year, climbing by $26 to $283.
For seniors wondering whether their payment has arrived, Medicare's helpline is 1-800-MEDICARE (1-800-633-4227). Payment status questions go to the Social Security Administration at 1-800-772-1213.
