For years, diet sodas and zero-sugar labels have carried a simple promise. No sugar means no harm. The evidence assembled by health authorities and researchers now tells a more complicated story, one worth understanding before reaching for that next can.
The World Health Organization drew a clear line in 2023. In its guideline on non-sugar sweeteners, WHO recommended against using them for weight control or to lower the risk of chronic disease. The systematic review behind the guidance found no long-term benefit for body fat in adults or children, and it flagged possible links to type 2 diabetes, cardiovascular disease, and mortality in adults.
"NSS are not essential dietary factors and have no nutritional value. People should reduce the sweetness of the diet altogether," said Francesco Branca, WHO Director for Nutrition and Food Safety.
The recommendation covers acesulfame K, aspartame, advantame, cyclamates, neotame, saccharin, sucralose, stevia and stevia derivatives. It applies to everyone except people with existing diabetes.

What new research adds on blood sugar
A 2026 meta-analysis pushes the picture further. Researchers at the Friedman School of Nutrition Science and Policy at Tufts University pooled 21 randomized clinical trials and found that non-nutritive sweeteners raised fasting insulin and HbA1c, a marker of long-term blood sugar control, when compared with non-caloric options like water or placebo.
The analysis in Current Atherosclerosis Reports reported a mean fasting insulin increase of 6.80 pmol/L across 16 trials and a modest HbA1c rise of 0.07% across 11 trials of at least two months.
The comparator choice matters. Earlier studies often measured sweeteners against sugar itself, which made the low-calorie versions look better by default.
By stacking them against calorie-free controls, the Tufts team isolated what the compounds themselves appear to do to metabolism.

"The rapidly increasing use of these sweeteners has outpaced our understanding of their long-term health effects. Until we know more, caution is needed," said senior author Dariush Mozaffarian, cardiologist and director of the Food is Medicine Institute at Tufts. He added that avoiding them where possible "appears a prudent choice," while noting they may still beat multiple daily servings of regular soda for people trying to cut back.
The gut microbiome question
Effects on the gut microbiome are real but far from uniform. A comprehensive review in the journal Diseases published in April 2025 by researchers led by Helal F. Hetta at the University of Tabuk summarized the current picture. Most of the strongest evidence comes from animal studies, and results vary widely between sweeteners and even between individuals.
In mice, sucralose has been tied to overgrowth of Proteobacteria and E. coli and to reduced butyrate-producing bacteria. Saccharin has produced dysbiosis in rodents, and human data show individual variation.
In one small human study, four of seven participants developed impaired glucose tolerance after saccharin exposure while the other three did not. Aspartame appears to alter oral and gut microbes in some studies but not in others. Stevia findings are inconsistent as well.

The review authors are cautious about jumping to conclusions. Many findings come from animal models, doses often exceed typical human intake, and commercial products contain bulking agents like maltodextrin that may drive some effects on their own.
The aspartame cancer question
In July 2023 the International Agency for Research on Cancer classified aspartame as possibly carcinogenic to humans, Group 2B, citing limited evidence for liver cancer. In the same evaluation, the WHO and FAO Joint Expert Committee on Food Additives reaffirmed the acceptable daily intake of 40 mg per kilogram of body weight.
For a 70-kilogram adult that works out to more than 9 to 14 cans of diet soda per day before exceeding the limit.
"Evidence of an association between aspartame consumption and cancer in humans is not convincing," said Moez Sanaa, head of the WHO unit on Standards and Scientific Advice on Food and Nutrition.

What it means in practice
None of this makes zero-sugar drinks poison, and the WHO guidance does not tell anyone to swap them back for regular soda. The direction of the evidence is narrower and more useful.
Treat non-sugar sweeteners as a bridge, not a destination. If they help someone cut out several daily servings of sugary drinks, that trade may still pay off. Assuming they are metabolically neutral, or safer than plain water over the long run, no longer fits what the trials are showing.
Anyone with existing diabetes, pregnancy, or ongoing metabolic concerns should raise the question with their doctor rather than rely on a food label.
