The nutrients your aging eyes need most – and the foods that deliver them

The National Institutes of Health's AREDS2 trial, a randomized study of 4,203 adults aged 50 to 85, found that adding lutein and zeaxanthin to an eye supplement lowered the risk of progressing to late age-related macular degeneration by 18 percent compared with beta carotene. The finding, published in JAMA Ophthalmology, has reshaped what doctors put in AMD supplements.

The two nutrients are the reason. Lutein and zeaxanthin are carotenoids, the yellow-to-orange pigments found in plants, and they are the only ones that build up in the macula, the small central patch of the retina responsible for reading, driving, and recognizing faces.

Why the retina hoards these two pigments

Macular pigment is made almost entirely of lutein, zeaxanthin, and a related isomer called meso-zeaxanthin. A review in the International Journal of Retina and Vitreous describes them as xanthophyll carotenoids that settle in the fovea, the pinpoint at the center of the macula, where they act as a filter and a resident antioxidant.

Once there, they do two jobs. They absorb blue light in the 400 to 500 nanometer range, cutting the high-energy wavelengths that damage retinal cells. And they act as antioxidants, quenching reactive oxygen species that build up in a tissue exposed to a lifetime of daylight.

Age-related macular degeneration is a slow breakdown of that same tissue. According to the CDC, roughly 19.8 million Americans were living with some form of AMD in 2019, and it remains a leading cause of severe, permanent vision loss in adults over 50.

What the AREDS2 trial actually showed

AREDS2 tested a 10 mg lutein and 2 mg zeaxanthin daily dose inside the standard AMD supplement, with a secondary analysis comparing it directly to the beta carotene arm. In that head-to-head, the hazard ratio for progression to late AMD was 0.82, and for the more aggressive neovascular form it was 0.78. The effect was strongest in participants with the lowest dietary intake of the two nutrients at baseline. The trial also gave researchers a reason to swap beta carotene out of the formula, because earlier work had linked it to a higher lung cancer risk in current and former smokers.

How to hit the target from the plate

A daily intake of about 6 mg of lutein and zeaxanthin has been proposed as a dietary target to lower AMD risk, and clinical work suggests 5 to 10 mg a day is a reasonable range. The average American gets only 1 to 3 mg.

Reaching the target from food is not hard. Cooked kale contains roughly 18 mg per 100 grams, raw spinach around 12 mg, and cooked spinach about 11 mg. Corn, orange peppers, and egg yolks add smaller amounts, and eggs punch above their weight because the fat in the yolk boosts absorption. That absorption point matters at every meal. Dressing a spinach salad with olive oil, or tossing kale with a drizzle of fat before roasting, meaningfully increases how much of the pigment the body takes up.

When a supplement enters the picture

For most healthy adults, food is enough. The AREDS2 formula is a different question. The National Eye Institute notes that certain vitamins can slow progression from intermediate to advanced AMD, and the formula is aimed squarely at people already diagnosed with intermediate disease or with advanced AMD in one eye. Anyone noticing blurred central vision, distorted lines, or a dark spot in the middle of the visual field should see an ophthalmologist rather than reach for a bottle.
