Pancreatic cancer's grim reputation is built on late diagnosis. The American Cancer Society's SEER-based figures show a 5-year relative survival rate of just 13% overall, but 44% when the disease is still confined to the pancreas.
The ACS estimates about 67,530 Americans will be diagnosed and 52,740 will die of the disease in 2026, making it one of the deadliest common cancers. The catch is that its early symptoms are subtle and easily blamed on ordinary gastrointestinal issues.
The 7 early warning signs
1. Jaundice and yellowing of the eyes or skin. A tumor at the head of the pancreas can block the bile duct, causing bilirubin to build up. According to the ACS, this often shows up alongside dark urine, light-colored or greasy stools, and itchy skin.
2. Unexplained weight loss. The ACS notes that unintended weight loss is very common in people with pancreatic cancer, often paired with poor appetite and no obvious cause.

3. Upper abdominal or mid-back pain. Pancreatic tumors can press on nearby organs, and when the cancer spreads to nerves around the pancreas it often causes back pain that does not shift with a change in position.
4. New-onset diabetes after age 50. Pancreatic tumors can destroy the insulin-making cells in the pancreas. The National Cancer Institute's Division of Cancer Prevention notes that 1 in 100 people with new-onset diabetes are diagnosed with pancreatic cancer within three years, and that 1 in 4 people who go on to be diagnosed with pancreatic cancer had first been diagnosed with diabetes. The signal is strongest in adults aged 50 to 85.
5. Pale, oily or floating stools. When bile and pancreatic enzymes cannot reach the intestines to break down fats, the ACS explains, stools can turn greasy and float in the toilet.
6. Nausea and loss of appetite. A tumor pressing on the stomach can trigger nausea or vomiting, and appetite tends to drop as the disease progresses.

7. Persistent fatigue. The ACS lists a lack of energy or physical weakness unrelated to physical activity as a common symptom in people with pancreatic cancer.
When two signs cluster, act sooner
Any single symptom on the list can have a benign explanation. What matters is persistence, and combinations. Zachary Stiles, DO, MS, a surgical oncologist at Roswell Park Comprehensive Cancer Center, told the center's CancerTalk that "these symptoms could mean a bile duct is blocked, which could indicate a tumor," and that two or more of these signs appearing together should prompt medical evaluation.
Pancreatic cancer accounts for roughly 3% of new US cancer cases but about 8% of cancer deaths. Because early tumors often produce no symptoms at all, any of these signs that lingers for more than a week or two, or that shows up alongside another, is a reason to speak to a doctor rather than wait it out. That is the same message doctors give for other cancers whose symptoms are easily brushed off.
