Adults with type 2 diabetes who started statin therapy within one year of their diagnosis had a 15% lower relative risk of developing dementia over 10 years, according to a Danish nationwide study of 132,585 people published in The Lancet Regional Health, Europe and presented at the European Society of Cardiology (ESC) Congress on 30 August 2026.
The finding is observational, meaning it can show a link but cannot prove that statins themselves prevent dementia. Patients who began treatment between one and five years after diagnosis had a 10% lower relative risk of dementia over 10 years, compared with those who never started a statin within five years.
Statins are cholesterol-lowering drugs that reduce low-density lipoprotein cholesterol (LDL-C), the type of blood fat most strongly linked to heart attack and stroke. They are already recommended for many adults with type 2 diabetes, whose baseline risk of cardiovascular disease is elevated.
What the Danish researchers did
The team, led by Dr. Tummas Ternhamar of Copenhagen University Hospital, pulled records from Danish registers for 132,585 statin-naïve adults newly diagnosed with type 2 diabetes between 2006 and 2019, and followed them through the end of 2021. The median follow-up was 7.1 years, and 2.7% of the group developed dementia in that time.

To reduce the bias that usually plagues observational research, the researchers used a clone-censor-weight design, a statistical method that mimics the structure of a randomized trial by comparing three matched groups: no statin within five years, statin started within one year, and statin started between years one and five.
"In individuals developing type 2 diabetes, statins were associated with a lower risk of dementia, most pronounced in those with early initiation," Ternhamar said. The results were similar for women and men.
Why cholesterol matters for the brain
The 2024 Lancet Commission on dementia prevention identified 14 modifiable risk factors that together account for close to 45% of dementia cases worldwide. High LDL cholesterol was one of two new risk factors added in the 2024 update, alongside untreated vision loss.
That backdrop matters because people with type 2 diabetes already carry both a heightened cardiovascular risk and a heightened dementia risk. Older adults with type 2 diabetes have roughly double the risk of dementia of those without diabetes, according to a pooled analysis of more than one million individuals.

Professor Isabel Goncalves, a member of the ESC Communication Committee who was not involved in the Danish study, put the finding in context. "People with type 2 diabetes already have a higher risk of dementia and are advised to lower cholesterol to reduce their risk of heart attack and stroke," she said. "Because statins are widely available and inexpensive, the findings are potentially important. However, this was an observational study, so it cannot prove that statins prevent dementia."
What to talk about with your doctor
If you have been newly diagnosed with type 2 diabetes and are not already on a statin, the study strengthens the case for asking whether one is right for you sooner rather than later. Ternhamar noted that statins "seem to be underused in patients with type 2 diabetes, despite their protective effects against cardiovascular disease."
Statins can cause side effects, including muscle pain that leads some people to stop treatment. Research has begun to identify possible biological causes of that muscle discomfort, and options such as switching statin type, adjusting the dose, or trying a different lipid-lowering approach are worth raising with a clinician if you have had trouble tolerating one.
The next step for confirming a causal link between statin timing and dementia risk would be a randomized controlled trial. Until then, cholesterol control after a type 2 diabetes diagnosis remains a standard cardiovascular recommendation, with a possible brain benefit now attached to acting early.
