Just 2.5 hours of moderate exercise a week may make your brain look nearly a year younger

A 12-month randomized trial in midlife adults put a specific number on that payoff.

Adults who followed the standard weekly exercise target for a year ended up with brains that looked measurably younger on an MRI scan than adults who did not, according to a randomized controlled trial from the AdventHealth Research Institute published in the Journal of Sport and Health Science.

The trial randomized 130 healthy adults between the ages of 26 and 58 to one of two groups. One group did two supervised 60-minute laboratory sessions a week plus home-based activity, totaling roughly 150 minutes of moderate-to-vigorous aerobic exercise a week. The other group carried on with usual care.

After 12 months, the exercise group's estimated brain age dropped by about 0.6 years. The usual-care group's brain age drifted up by about 0.35 years. The gap between the groups worked out to close to a full year (mean difference -0.95 years, 95% confidence interval -1.72 to -0.17, p = 0.019).

What "brain age" on an MRI actually means

Brain age is a biomarker built from MRI scans. A machine-learning model is trained on scans from thousands of healthy people so it learns what a typical brain looks like at each chronological age. When it then reads a new scan, it estimates the person's brain age from the structure alone. The difference between that predicted brain age and the person's real age is called brain-PAD, or brain predicted age difference.

A positive brain-PAD suggests the brain looks older than the person actually is, which prior research links to worse cognitive performance and higher risk of later health problems. A negative brain-PAD points in the other direction. In this trial, exercise pushed the number the healthier way.

A guideline dose, not a punishing routine

The exercise target the researchers used is not extreme. It matches the CDC's guideline of 150 minutes of moderate-intensity aerobic activity a week for adults, which can be broken up into roughly 30 minutes on five days.

"We found that a simple, guideline-based exercise program can make the brain look measurably younger over just 12 months," said Dr. Lu Wan, lead author and data scientist at the AdventHealth Research Institute, in a press release from the institution. She called the absolute changes modest but noted that even a one-year shift could matter over decades.

One finding surprised the team. Improvements in cardiorespiratory fitness, blood pressure, body composition and BDNF, a growth factor linked to brain health, did not statistically explain the brain-age effect. Wan said exercise may be acting through additional mechanisms the trial did not capture, such as subtle changes in brain structure, inflammation or vascular health.

Why midlife matters

Senior author Dr. Kirk I. Erickson, director of translational neuroscience at the AdventHealth Research Institute, framed the timing as important. "Intervening in the 30s, 40s and 50s gives us a head start," Erickson said in the press release. "If we can slow brain aging before major problems appear, we may be able to delay or reduce the risk of later-life cognitive decline and dementia."

A few caveats are worth carrying with the number. This is one randomized trial in 130 people, not a settled truth about the whole population, and the effect on any individual scan is small. The brain-age model estimates a group-level pattern, not a personal verdict. What the results do show is that a standard weekly exercise dose was enough to move a structural brain marker in a healthier direction over a single year in midlife adults.

The practical takeaway lines up with what public-health agencies already recommend. Adults meeting the 150-minute weekly target through brisk walking, cycling or similar activities are, on the evidence of this trial, giving their brain structure a measurable nudge in the younger direction.
