Your waist measurement may say more about your air pollution risk than the number on the scale. Adults with extra fat packed around their internal organs appear far more vulnerable to ultrafine pollution than leaner people.
The finding comes from a randomized crossover trial published on July 28, 2026 in the American Journal of Preventive Cardiology.
Researchers at the University of Connecticut and Tufts University followed 116 adults living within 200 meters of Interstate 93 north of Boston.
Participants used a real HEPA air purifier for 30 days and a sham filter for another 30, in random order. Blood samples tracked proteins tied to inflammation and cardiovascular disease.

What the trial actually showed
Across the whole group, filtering the indoor air made no meaningful difference on average. But when the team sorted participants by waist size relative to height and weight, a clear pattern emerged.
Adults with the most visceral fat, the deep abdominal fat that wraps around organs like the liver, showed a strong drop in the inflammatory marker IL-1 beta while the real HEPA unit was running compared with the sham filter.
Similar benefits appeared in older participants and those with elevated blood pressure. BMI, which lumps muscle and fat together, did not predict who benefited.
Leaner participants did not show the same reduction in the inflammatory marker, according to the trial results reported by Doug Brugge, chair of public health sciences at the University of Connecticut School of Medicine.

Why the belly matters more than the scale
Ultrafine particles, defined as pieces of pollution smaller than 100 nanometers across, are the smallest and most invasive fraction of what people breathe.
They slip from the lungs into the bloodstream and reach the heart and brain, where research links them to cardiovascular disease, dementia, hypertension and respiratory illness.
Visceral fat is not passive storage. It releases cytokines like IL-1 beta that fuel chronic low-grade inflammation, and the trial suggests it also primes the body to react more strongly when pollution reaches the blood.
That helps explain why waist size can catch health risks that a normal BMI hides.

What the numbers look like
The National Heart, Lung, and Blood Institute treats a waist above 40 inches in men and above 35 inches in women as a marker of increased risk for heart disease and type 2 diabetes. The trial suggests those same numbers may also flag who has the most to gain from cleaner indoor air.
Measuring at home is simple. Wrap a soft tape around the bare abdomen just above the hip bones and read the number at the end of a normal breath out.
Brugge and Tufts biostatistician Misha Eliasziw are now running a follow-up trial in Hartford and Boston. It tests a cheaper alternative to commercial HEPA units, a box fan strapped to four furnace filters, an assembly popularized during the pandemic.
