First-borns and second-borns are not equally likely to end up with the same diagnoses on their medical charts.
A new analysis of health records from more than 10 million US siblings identified 150 conditions where the odds of a diagnosis shift with birth order.
The study, published in Nature Health, drew on commercial insurance claims from the Merative MarketScan database, covering 5.1 million two-child families.
Researchers screened 569 disease categories, and 150 met their statistical thresholds after adjustments.

The differences are modest at the level of any single family. But because birth order applies to almost every person on Earth, even small shifts add up across a population.
Where first-borns and second-borns diverge
First-borns were more often diagnosed with a cluster of neurodevelopmental and immune-allergic conditions.
That included autism, ADHD, tics and Tourette syndrome, obsessive-compulsive disorder, depression and anxiety, along with asthma, allergic rhinitis, food allergies, and acne, according to reporting by Newsweek on the same paper.
Second-borns tipped the other way. They showed higher recorded odds for substance use disorders, migraines, gastritis and other gastrointestinal issues, biliary tract disease, kidney infections, and shingles.

Why birth order might matter
The researchers point to several overlapping mechanisms rather than one cause.
The best-known is the hygiene hypothesis, the idea that early microbial exposure trains the immune system away from allergy and asthma.
Younger siblings, who grow up alongside an older child bringing home germs from daycare and school, get that exposure earlier and more often.
Parental age at birth, the length of pregnancy, and differences in how closely parents monitor a first versus a second child are also on the researchers' list of possible drivers. Neurodevelopmental diagnoses, in particular, likely reflect "a mixture of pregnancy-order biology, parental surveillance, diagnostic timing," the authors write.

Several links weakened as the age gap between siblings widened.
Food allergies, allergic rhinitis, and substance use disorders all showed smaller birth-order effects when siblings were spaced further apart, according to the paper and coverage in News Medical.
What the numbers do and do not say
The analysis is large, but it looks at diagnoses recorded during insured healthcare visits, not at true disease incidence.
Uninsured Americans and Medicaid enrollees were not in the dataset. Every association reported is statistical, and the researchers stress they have not established causation.

For a parent or a patient, that means birth order does not predict what will happen to a particular sibling. It nudges group averages.
Anyone worried about symptoms of autism, ADHD, asthma, allergies, or any of the other conditions on the list should raise them with their doctor rather than reading a health trajectory off the family tree.
The paper covers only two-child families, and the effects it reports are averages across millions of records.
Whether the same patterns show up in larger families, in different countries, or in datasets that include uninsured patients is still open.
