If you take a daily thyroid pill and have started a GLP-1, the order you swallow them in matters more than you might think.

If you take a thyroid pill every morning, you probably have a tight routine around it.

Add a new weight-loss drug to the mix, and the question gets harder. When do you take what, and does it actually matter?

For patients juggling both, new clinical guidance from UT Southwestern Medical Center addresses two of the biggest worries.

The first is about cancer risk. The second is a thirty-minute window that most people on both drugs have never been told about.

Jeffrey Zigman, M.D., Ph.D., is a professor of internal medicine at UT Southwestern. He points to a large study from Denmark, Norway and Sweden that found no substantial rise in thyroid cancer among patients on GLP-1 drugs. The follow-up averaged nearly four years.

The cancer concern

GLP-1 medications include semaglutide, the active ingredient in Novo Nordisk's Ozempic and Wegovy, and orforglipron, the active ingredient in Eli Lilly's Foundayo oral pill.

All of them carry a warning tied to medullary thyroid carcinoma, a rare cancer of the C cells in the thyroid gland.

The warning was triggered by animal studies in which the drugs made these cells grow.

The Scandinavian registry study, published in The BMJ in March 2024, compared 145,410 adults on GLP-1 drugs with 291,667 adults on a different diabetes drug class.

The researchers found no higher risk of thyroid cancer in the GLP-1 group.

The reassurance does not apply to a rare inherited condition called multiple endocrine neoplasia type 2, in which about 95 percent of patients carry a high risk of medullary thyroid cancer.

People with that diagnosis, or a close family history of it, should avoid GLP-1 drugs.

Hashimoto's disease, hypothyroidism, goiter and the common papillary and follicular thyroid cancers are not on the warning list. Zigman notes that this covers the vast majority of thyroid patients.

The 30-minute rule for the oral pill form

The practical headache for patients on both medications sits with the oral forms. Thyroid hormone replacement, levothyroxine, has to be taken on an empty stomach for the body to absorb it properly.

The oral version of Wegovy, approved by the FDA in December 2025, works the same way.

The guidance for people on both oral Wegovy and levothyroxine goes like this. Swallow the oral Wegovy right after waking with no more than 4 ounces of plain water. Wait 30 minutes. Take the levothyroxine. Wait another 30 minutes before eating.

Foundayo, the orforglipron pill cleared by the FDA on April 1, 2026, works differently. It has no food or water restrictions. Patients can take their levothyroxine first thing in the morning, wait 30 minutes before breakfast, and then take Foundayo with their other medications.

Weekly injectable GLP-1s like Ozempic, Wegovy and Mounjaro do not interfere with the morning thyroid pill.

When weight loss means your thyroid dose needs a tweak

Thyroid hormone doses are set in part by body weight. Lose a lot of weight on a GLP-1 drug, and the dose that fit you a year ago may now be too high.

UT Southwestern recommends a conversation with your clinician after any swing of 10 to 20 pounds. Blood tests of TSH and free T4 can confirm whether the dose still fits.

Common GLP-1 side effects such as nausea, constipation and reflux can feel like thyroid trouble when they first appear. The guidance recommends checking TSH six to eight weeks after a 5 to 10 percent change in body weight.
