Helicobacter pylori is one of the most common bacterial infections in the world, and one of the quietest. The International Agency for Research on Cancer classifies it as a Group 1 carcinogen, meaning there is sufficient evidence that it causes cancer in humans.

Roughly two-thirds of the global population carries it, and in the United States prevalence rises sharply with age, from about 10 to 15 percent among people in their late teens to as high as 50 to 67 percent in adults over 60, according to NIDDK data.

The strange part is how silent it is. Most people who are infected have no symptoms at all, which is exactly why untreated H. pylori can linger for years and quietly raise the risk of peptic ulcers and stomach cancer. When symptoms do appear, they look a lot like ordinary indigestion, and that is where the trouble starts.

The 7 warning signs to know

The hallmark sign is a dull or burning pain in the upper abdomen. Cleveland Clinic describes it as pain that often shows up a few hours after eating and at night, lasting minutes to hours and coming and going over days or weeks. Penn Medicine adds that the ache is often worse on an empty stomach.

Beyond that lead symptom, doctors flag six more warning signs commonly linked to H. pylori infection:

None of these are unique to H. pylori. They overlap heavily with acid reflux, everyday indigestion, and stress-related stomach upset, which is why the infection is so often missed.

When to ask a doctor about testing

If several of these symptoms show up together, are getting worse, or keep coming back over more than a couple of weeks, that is the moment to raise H. pylori with a clinician.

Certain signs are more urgent. Penn Medicine lists bloody or dark, tarry stools, bloody vomit, severe blood loss, or a sudden severe stomach pain among reasons to seek care quickly, because they can point to a bleeding or perforated ulcer.

Testing is straightforward and non-invasive in most cases. A urea breath test asks the patient to swallow a substance containing urea, which H. pylori converts to carbon dioxide that shows up on the breath. A stool antigen test detects the bacterium directly in a stool sample.

When symptoms are more serious, doctors may look inside the stomach with an upper endoscopy and take a small tissue sample.

What treatment involves

The standard approach is a combination of antibiotics with a proton pump inhibitor, a drug that lowers stomach acid, taken for roughly 10 to 14 days. Bismuth is sometimes added to the mix. It is a demanding course to finish, but it works, and eradicating the bacterium removes the underlying driver of ulcers and lowers long-term cancer risk.

The National Cancer Institute notes that most cases of gastric adenocarcinoma and of gastric MALT lymphoma are attributed to H. pylori infection, and that in long-term follow-up, treating the infection has been linked to a substantial drop in stomach cancer incidence.

That is the practical case for taking the symptoms seriously rather than shrugging them off as indigestion. Anyone who recognizes several of the signs above should talk to a doctor about a breath or stool test.
