How hard you can squeeze a handheld gauge may tell you more about your future than the number on your blood pressure cuff.
That is the striking finding from the Prospective Urban Rural Epidemiology (PURE) study, a prospective analysis of 139,691 adults across 17 countries published in The Lancet. Researchers led by Darryl Leong at McMaster University tracked participants for a median of four years and found that grip strength was a stronger predictor of both all-cause and cardiovascular mortality than systolic blood pressure.
Each 5-kilogram (about 11-pound) drop in grip strength was associated with a 16 percent higher risk of death from any cause and a 17 percent higher risk of cardiovascular death, according to the analysis. The authors described grip strength as a simple, inexpensive risk-stratifying method for all-cause death, cardiovascular death, and cardiovascular disease.
A separate meta-analysis published in the Journal of the American Medical Directors Association in 2017 pooled data from 42 prospective studies and more than 3 million participants. People in the lowest grip-strength category had a 41 percent higher risk of death from any cause than those in the highest category, the pooled analysis found, along with a 63 percent higher risk of cardiovascular disease.

Why a squeeze can say so much
Grip strength is more than a measure of forearm muscle. It reflects the accumulated state of the muscular, cardiovascular, and nervous systems.
"Hand grip strength is one of the most overlooked indicators of long-term health. With a single squeeze, it reflects the accumulated effects of how we move, fuel, recover, and live," Pete Rohleder, a kinesiologist at Kansas State University, told National Geographic in a January 2026 report.
The measure captures signals from several biological pathways at once. It correlates with vascular health and cardiorespiratory fitness, autonomic nervous system function, and sarcopenia, the age-related loss of muscle mass that quietly erodes independence. Grip strength has also been linked to markers of biological aging such as DNA methylation and to brain structure integrity, National Geographic reported.
Harvard Health cautions that no evidence yet shows that building forearm muscles will directly extend life. What a stronger grip does, the publication notes, is support the everyday tasks, opening jars, carrying groceries, holding a cane, that help older adults stay independent longer.

Four exercises to build a stronger grip
Writing for Harvard Health Publishing in November 2024, Sarah Klein outlined four simple moves to strengthen the forearms and hands. Start with one set of 10 repetitions and build to two or three sets over time.
Wrist extension and flexion. Hold light dumbbells (1 to 3 pounds) at your sides with elbows bent and palms facing down. Lift the weights upward using only the wrist until you feel a gentle stretch on the inside of the wrist, hold for five counts, then lower them until you feel a stretch on the outside. Ten repetitions.
Wrist supination and pronation. With the same light dumbbells in hand and elbows bent, rotate your forearm so the palm faces up, hold for five counts, then rotate so the palm faces down. Continue for 10 repetitions.
Putty grip. Roll a piece of therapy putty into a ball, place it in your palm and gently squeeze and release. Repeat 10 times on each hand every other day.

Putty three-point pinch. Shape the putty into a short, thick cylinder and place it between the pads of your thumb and your index or middle finger. Gently squeeze and release 10 times per hand every other day.
A farmer's carry, walking about 20 feet while holding a moderate dumbbell or kettlebell in each hand, is another accessible option that builds grip alongside core and shoulder stability.
Anyone with joint pain, arthritis or a recent hand or wrist injury should talk to a physician or physical therapist before starting a new grip routine.
