The cholesterol marker most doctors don’t routinely test may be raising your heart risk

A large new analysis has tied very high levels of a genetic cholesterol particle called Lp(a), short for lipoprotein(a), to a sharply higher risk of stroke and cardiovascular death.

The particle sits outside the standard lipid panel your doctor orders at a checkup, so millions of adults with normal LDL results are carrying elevated risk without ever seeing it on paper.

The findings, presented in April at the Society for Cardiovascular Angiography and Interventions 2026 meeting in Montreal, come from pooled data on 20,070 adults aged 40 and older enrolled in three large NIH trials, ACCORD, PEACE and SPRINT.

Over a median follow-up of about four years, 7.3 percent of the participants had a major cardiovascular event.

What the study actually found

Adults whose Lp(a) topped 175 nmol/L had a 31 percent higher risk of major adverse cardiovascular events, a 49 percent higher risk of cardiovascular death and a 64 percent higher risk of stroke compared with those below that level. No significant link was seen for heart attack in this analysis.

Because the study is observational, it points to a strong association, not proof of cause.

The signal was more pronounced in people who already had cardiovascular disease, according to reporting on the presentation by tctmd.com.

Why an ordinary cholesterol test misses it

Lp(a) is a specific form of LDL cholesterol with an extra protein attached that promotes blood clotting and inflammation in artery walls.

The Cleveland Clinic notes that Lp(a) "is not part of a standard lipid panel that checks the kinds of cholesterol everyone talks about," and that levels are set almost entirely by the genes a person inherits from their parents and remain fairly stable for life.

That is why diet, exercise and statins, which lower ordinary LDL, do little to move Lp(a). Roughly one in five people worldwide carries elevated Lp(a), the Cleveland Clinic estimates, and most have no idea.

How to get your Lp(a) checked

The test itself is a simple blood draw. Ask your primary care doctor or cardiologist to add lipoprotein(a) to your next lab order.

In the United States it is billed under CPT code 83695, a number worth mentioning if you are checking insurance coverage in advance.

National heart guidelines now support measuring Lp(a) at least once in adulthood, since a single result generally reflects lifetime levels.

The lead investigator on the new analysis, Dr. Subhash Banerjee of Baylor Scott & White in Dallas, told tctmd.com, "My hope is that [Lp(a)] becomes part of routine testing when patients first come to the cath lab."

Who should ask first

The case for testing is strongest for adults with a family history of early heart disease or stroke, anyone diagnosed with familial hypercholesterolemia, and people who have already had a cardiac event despite normal cholesterol numbers, according to the Cleveland Clinic.

A high Lp(a) result does not currently have an approved drug to lower the particle directly, though several are in late-stage trials. In the meantime, doctors typically respond by pushing LDL cholesterol lower and managing blood pressure, smoking and diabetes more aggressively. Anyone concerned about their own heart risk, especially with a strong family history, should raise Lp(a) testing with their doctor.
