The Cooper 12-minute run test asks one thing. Cover as much ground as you can on foot in 12 minutes. Dr. Kenneth Cooper, then a US Air Force flight surgeon, developed the test in 1968 as a fast, low-cost way to gauge aerobic fitness in airmen. His original work, published in JAMA, reported a strong correlation between the distance covered and lab-measured VO2 max.

The test endures because the number closely tracks VO2 max, the maximum amount of oxygen your body can use during hard exercise. A 2016 systematic review and meta-analysis in PLOS ONE pooled 123 studies and 200 correlation values and found the 12-minute walk/run had one of the highest criterion-related validities among field tests, with a pooled correlation of 0.78.

What a healthy distance looks like

Published norms sort results by age and sex.

For men aged 20-29, an excellent result is more than 2,800 meters, above average is 2,400-2,800, and average sits at 2,200-2,399. For men 30-39, excellent is above 2,700 meters and average is 1,900-2,299. For men 40-49, excellent starts above 2,500 meters and average is 1,700-2,099. For men 50 and older, excellent begins above 2,400 meters and average is 1,600-1,999, per the Cooper test reference table.

For women aged 20-29, excellent is above 2,700 meters and average is 1,800-2,199. For women 30-39, excellent begins above 2,500 meters and average is 1,700-1,999. For women 40-49, excellent is above 2,300 meters and average is 1,500-1,899. For women 50 and older, excellent begins above 2,200 meters and average is 1,400-1,699.

Why this number matters beyond running

VO2 max is one of the strongest independent predictors of long-term health. A 46-year follow-up of 5,107 middle-aged Danish men, published in the Journal of the American College of Cardiology in 2018, found that each one-unit increase in VO2 max was associated with about 45 additional days of life expectancy, with similar estimates for cardiovascular mortality.

That is an association, not proof that running further guarantees more years. Fitness reflects many things at once, including heart health, muscle function, and daily activity habits. Even so, higher cardiorespiratory fitness consistently tracks lower mortality across large cohort studies.

A safe way to test yourself

The protocol is straightforward. Warm up for at least 10 minutes with easy jogging and light drills. Head to a measured 400-meter track or a flat, marked path. Start a timer and cover as much distance as you can in 12 continuous minutes. Walking is allowed if you cannot keep running the whole time.

The American College of Sports Medicine recommends that adults get 30 minutes of moderate aerobic activity five days a week, or 20 minutes of vigorous activity three days a week. If you are new to running, live with a chronic condition, or are over 40 and have been inactive, check with your doctor before pushing to an all-out effort.
