In a randomized trial at Stanford Medicine, healthy adults who spent 10 weeks eating yogurt, kefir, fermented cottage cheese, kimchi, fermented vegetables, vegetable brine drinks and kombucha showed a broader mix of gut bacteria and a drop in 19 inflammatory proteins in their blood.
The comparison arm ate a high-fiber diet, long promoted as a top gut-health strategy. Those participants did not see the same fall in inflammatory markers, and their microbial diversity did not budge.
One of the proteins that dropped was interleukin-6, a signaling molecule the researchers describe as a key driver of chronic inflammation and one that has been linked to rheumatoid arthritis, type 2 diabetes and chronic stress. Lower background levels of these proteins are what scientists call an anti-inflammatory shift.
The trial was small. Only 36 healthy adults completed it, split between the two diets, and the authors call for replication in larger groups before broader guidance is drawn. The study was published in Cell in July 2021 by researchers at Stanford University School of Medicine.

What actually happened in the trial
Participants were randomly assigned to one of the two diets and worked up gradually to a target level. On the fermented arm, the target was six servings a day by the end of the intervention. The observation period ran 3 weeks before the diet started and 4 weeks after it ended, so researchers could see how quickly changes appeared and how they held.
Stool and blood samples showed a dose-response pattern in the fermented group. The more fermented food a person ate, the more diverse their gut microbiome became, and the lower their inflammatory signals ran. Four types of immune cells also showed reduced activation.
"This is a stunning finding," Justin Sonnenburg, an associate professor of microbiology and immunology at Stanford and co-senior author, said in the university's announcement. "It provides one of the first examples of how a simple change in diet can reproducibly remodel the microbiota across a cohort of healthy adults."
The fiber result surprised the team. Erica Sonnenburg, a senior research scientist and co-senior author, said the group had expected the opposite. "We had expected high fiber to have a more universally beneficial effect and increase microbiota diversity," she said. "The data suggest that increased fiber intake alone over a short time period is insufficient to increase microbiota diversity."

What it may mean for your plate
Fermented foods are not new, and none of this makes them a cure for anything. What the trial suggests is that a regular daily habit, not an occasional spoonful of yogurt, is what shifted the numbers. Six servings a day is a lot, but the researchers reported effects even at lower amounts along the way.
The list is also broader than people often assume. Alongside yogurt and kefir, the study used kimchi, sauerkraut-style fermented vegetables, kombucha and vegetable brines. Cottage cheese counted only when it was a live-culture, fermented version.
Fiber still matters. This trial did not show fiber to be harmful, and other research continues to tie fiber-rich diets to better long-term health. It suggests the two food groups may act through different routes in the gut, and that fermented foods deserve a place alongside fiber rather than a swap.
The 2021 findings are now being tested elsewhere. A community trial in rural Pakistan published in Scientific Reports in 2025 looked at fermented pickles in women of reproductive age and reported changes in gut microbiota and immune parameters, adding early support in a very different population. Larger and longer trials are still needed before firm clinical recommendations can be made, and anyone with a chronic inflammatory condition should talk to their doctor before making big dietary changes.
