Millions of people wake up to a heart rate variability score on their Fitbit, Apple Watch, WHOOP or Garmin. Few know what the number means, and even fewer know how it should shift as they age.

Peer-reviewed research from the Frontiers in Physiology dataset of nearly 14,000 healthy ECGs and a Korean aging study in Frontiers in Aging Neuroscience shows that heart rate variability drops steadily from your 20s onward, and that the drop is normal.

Heart rate variability, or HRV, is the tiny fluctuation in the gap between one heartbeat and the next, measured in milliseconds. It is not the same as your pulse.

It reflects the balance between the two arms of your autonomic nervous system, the Cleveland Clinic explains: the sympathetic branch that drives your fight-or-flight response, and the parasympathetic branch that governs rest and recovery. A higher HRV generally signals a body that switches between the two smoothly.

What a normal HRV looks like at your age

The clearest age-banded numbers come from the Korean study, which measured SDNN, a common HRV metric, in 291 healthy adults. Values are means in milliseconds, plus or minus one standard deviation.

The Korean team reported that every HRV index they measured, including SDNN, RMSSD, pNN50 and the frequency bands, declined with age at p less than 0.001. Sex differences were small and mostly not statistically meaningful within age bands.

A larger dataset points in the same direction. The Frontiers in Physiology analysis of 13,943 ECGs from cardiologically healthy people aged 11 days to 91 years found that median heart-rate-corrected SDNN fell from roughly 48 to 56 ms in the 20-29 group to about 20 to 21 ms in the 60-69 group. The authors noted that the median and the lower limit of normal decreased steadily from birth until old age.

Why HRV falls with age

The autonomic nervous system loses some of its flexibility over the decades. Parasympathetic tone, the rest-and-recovery signal that mostly drives beat-to-beat variability, weakens. That is why a 25-year-old and a 65-year-old with the same resting pulse can have very different HRV scores.

Age is not the only lever. The Cleveland Clinic lists genetics, sex, fitness level, sleep, stress, alcohol, medications such as beta-blockers, pacemakers and illnesses including diabetes, hypertension, arrhythmia, asthma, anxiety and depression as factors that shape the number on your wrist.

That is one reason two people the same age can score 20 ms apart on the same morning.

What a low HRV may signal

Persistently low HRV is one of the more consistently studied cardiovascular red flags in the research literature. A proof-of-concept analysis of 9,550 working adults published in 2019 reported that a daytime RMSSD value at or below 25 ms was linked to elevated risk across cardiovascular markers including blood pressure, LDL cholesterol, fasting glucose, HbA1c and C-reactive protein, with odds ratios between 1.35 and 3.45.

Low HRV has also been linked to the future development of high blood pressure in people who do not yet have it, and to higher rates of hospitalized heart failure and sudden cardiac death in large population studies. These are associations, not proof of cause, and no wearable is a diagnostic device.

The practical read is simple. A single low morning is normal after a late night, a hard workout, alcohol or a cold.

If your HRV trends lower than usual for a week or two without an obvious reason, especially if it comes with fatigue, chest symptoms, palpitations or new blood pressure readings, that is a conversation to have with your doctor rather than your app. This is the same guidance the Cleveland Clinic gives on interpreting the number, and it lines up with our earlier reporting on what your resting heart rate can reveal about long-term risk.

How to read your own number

Compare yourself with yourself, not with someone else's screenshot. Wearables use different sensors, different windows and different formulas, so a WHOOP number and an Apple Watch number are not directly interchangeable.

Track a rolling average over weeks. A gradual rise usually tracks better sleep, more aerobic training and lower alcohol intake. A gradual fall usually tracks the opposite.
