Sleeping under a weighted blanket may genuinely calm anxiety and improve sleep quality, according to a randomized controlled trial of 120 psychiatric patients published in the Journal of Clinical Sleep Medicine in September 2020.
A 2024 pilot trial in 102 adults with insomnia, published in BMC Psychiatry, adds fresh evidence that the effect extends beyond psychiatric wards to ordinary poor sleepers.
The trial from Karolinska Institutet in Stockholm used metal chain-weighted blankets. Most patients slept under an 8-kilogram (about 17.6-pound) version, about 10 percent of body weight, for four weeks.
Participants who used the weighted blanket were almost 26 times more likely than the control group to see their insomnia severity drop by half or more, and nearly 20 times more likely to reach remission.

Self-rated fatigue, anxiety and depression during the day all fell significantly compared with a light plastic chain control blanket weighing 1.5 kilograms.
Where the calming effect may come from
The proposed mechanism is deep pressure stimulation. The gentle, even weight distributed across the body appears to activate the parasympathetic nervous system, the branch that slows the heart and shifts the body into a rest state.
A 2024 review in Frontiers in Psychiatry describes deep pressure touch as the most widely accepted mechanism among researchers, and notes it has been linked to the release of endorphins, dopamine and serotonin.
One study cited in the review also found that weighted blankets raised salivary melatonin before sleep in young, healthy adults.

Cleveland Clinic recommends choosing a blanket no heavier than 10 percent of your body weight so it does not feel oppressive.
Dr. Michelle Drerup, a sleep psychologist quoted in the same article, points out that some studies link weighted blankets to reduced anxiety, though she adds they should not be a frontline treatment for nighttime anxiety.
What the second trial actually found
The 2024 BMC Psychiatry pilot randomized 102 adults with clinical insomnia across three Chinese hospitals to either a 5.5-kilogram weighted blanket or a 3.5-kilogram normal blanket for one month.
After the intervention, the weighted-blanket group improved more on the Pittsburgh Sleep Quality Index, with a mean drop of 4.1 points versus 2.0 points in the control group.

Daytime sleepiness, stress, anxiety, fatigue and bodily pain all improved significantly in the weighted-blanket group, while insomnia severity scores did not show a significant between-group difference.
The results are consistent with the earlier Karolinska trial in one important way. Both point to sleep-related daytime symptoms, tiredness and anxious feelings, as areas where the blanket seems to help most.
Who should skip the weighted blanket
Weighted blankets are not for everyone. The Frontiers review notes that children under 3 years of age or weighing less than 50 pounds should not use them because of the risk of suffocation or entrapment.
Elderly people with frailty, limited mobility or severe dementia should talk with a physician first and use the lightest option available.

People with claustrophobia may feel panicked rather than soothed under the added pressure, and anyone with a serious respiratory condition such as sleep apnea, asthma or chronic lung disease may find that chest pressure worsens breathing at night.
For most healthy adults with garden-variety trouble sleeping or daytime anxiety, the current evidence suggests a weighted blanket is a low-risk experiment worth trying.
If sleep does not improve after a few weeks, or if anxiety is persistent and disruptive, a conversation with a doctor is the right next step.
