A 2025 study finds high salt intake damages kidney tissue, even without raising blood pressure.

Most people think of a salty diet as a blood-pressure problem. A new experimental study points to something more direct.

High salt intake appears to leave structural marks on the kidney itself, and it can begin before pressure readings climb.

The research, published in Frontiers in Physiology in September 2025, fed rats a high-salt diet and then examined their tissues under the microscope.

The animals showed glomerular hypertrophy (enlarged filtering units), tubular dilatation, and a large jump in interstitial collagen, a marker of early fibrosis.

These changes appeared without a significant rise in systemic blood pressure.

What scarring looks like in the kidney

The kidney filters blood through roughly a million tiny units called glomeruli.

When those units are pushed to work harder, or when scar tissue collects between them, filtration efficiency drops.

That accumulation of extracellular matrix, mostly collagen, is what pathologists mean by renal fibrosis. It is a central step on the road to chronic kidney disease.

The authors concluded that a high-salt diet produced early, region-specific tissue changes in both the aorta and the kidney, appearing before blood pressure climbed. Because this was an animal study, it cannot prove the same timeline in people, but it fits a wider pattern from human epidemiology.

The stroke number people miss

Human data on sodium have long focused on stroke, and the numbers are sobering.

In the INTERSTROKE case-control study of nearly 19,000 people across 27 countries, participants who excreted more than 4.26 grams of sodium a day (roughly 11 grams of salt) had 81 percent higher odds of stroke compared with a reference intake of 2.8 to 3.5 grams.

The odds were higher still for hemorrhagic stroke, where bleeding occurs inside the brain.

Kidney damage tracks the same curve. A 2025 analysis in Frontiers in Nephrology found that deaths from chronic kidney disease attributable to high sodium climbed 1.68-fold worldwide between 1990 and 2021, from roughly 26,000 to nearly 70,000 a year.

Where all the salt comes from

The World Health Organization advises adults to stay under 2,000 mg of sodium a day, about a teaspoon of salt. Global intake averages 4,278 mg, more than double the target.

WHO links about 1.7 million deaths a year to excess sodium.

American adults sit close to that global figure. The average intake in the United States is around 3,400 mg of sodium daily, and the American Heart Association recommends no more than 2,300 mg, with an ideal ceiling of 1,500 mg for most adults.

Most of that sodium is not the shaker on the table. According to federal dietary data, roughly three quarters comes from packaged and restaurant foods.

The biggest categories are sandwiches, pizza, cold cuts and cured meats, soups, and breads and rolls.

Reading labels on bread, deli meats, and canned soups, and choosing versions with lower sodium per serving, is where most of the reduction lives.

Anyone with existing kidney disease, high blood pressure, or heart failure should discuss a specific sodium target with their doctor.
