Adding omega-3s to a weight-loss plan may lower breast cancer risk in high-risk women

Women at elevated risk for breast cancer who paired a behavioral weight-loss program with high-dose omega-3 fatty acids showed measurable improvements in key breast cancer risk markers, according to a pilot study published in the August 2026 issue of Cancer Prevention Research, an American Association for Cancer Research (AACR) journal.

The 34 peri- and postmenopausal women in the trial had a body mass index of at least 28. Half were randomized to 3.25 grams a day of omega-3 fatty acids and half to placebo, alongside the weight-loss program. The median weight loss was around 10 percent of starting body weight.

This is a biomarker study, not a cancer incidence trial. It tracked markers linked to breast cancer risk, not new cancer cases, so any effect on actual risk remains an inference from those markers rather than proof.

Which markers improved

Among women who lost at least 10 percent of their body weight, those on omega-3 had the largest drop in the Firmicutes-to-Bacteroidetes ratio, a broad measure of gut bacterial balance. Their samples showed more of two short-chain fatty acid producing microbes, Phocaeicola vulgatus and Alistipes putredinis, and higher blood levels of propionate, a fatty acid these bacteria make when they ferment fiber.

Several circulating markers tied to breast cancer risk moved in a favorable direction. Bioavailable estradiol dropped, as did C-reactive protein, a general marker of inflammation. The ratio of adiponectin to leptin, two hormones released by fat tissue, improved.

Why the biology may matter

After menopause, most of a woman's estrogen comes from fat tissue rather than the ovaries. Fat tissue contains an enzyme called aromatase, which converts other hormones into estrogen. That is one reason the American Cancer Society lists being overweight or obese after menopause as an established breast cancer risk factor, and why lowering circulating estrogen through weight loss is thought to matter.

Weight loss alone has already been tied to a lower risk. Breastcancer.org notes that losing as little as 10 to 15 pounds may lower breast cancer risk for at least a decade in postmenopausal women with obesity. The new trial suggests omega-3 supplementation may amplify some of the biological changes behind that link, at least in the short term.

What omega-3 is, and the honest caveats

EPA and DHA, the two long-chain omega-3 fats found in fatty fish and fish oil capsules, have been studied for years as possible cancer-risk modifiers. Prior research is mixed. A 2015 review in Breast Cancer Research described omega-3 as a promising candidate for breast cancer prevention and survivorship, but concluded that clinical evidence is still limited and inconsistent.

The 3.25 grams used in the trial is far higher than the amount in a standard fish-oil capsule and closer to a therapeutic dose. High-dose omega-3 can thin the blood and interact with anticoagulants and some other medications, so women considering it, particularly those already in cancer care, should raise it with their oncologist or primary care doctor before starting.

The trial included only 34 women and lasted long enough to track biomarker shifts, not enough to say whether fewer cancers actually followed. Larger, longer trials are needed to test whether the combination changes cancer outcomes and not just the markers that predict them.
