Ice baths and cold plunges have moved from elite locker rooms to everyday wellness routines, with millions of people dipping daily for benefits that range from faster muscle recovery to better metabolism, mood and immunity.
The science, though, is uneven. One claim is solidly backed by controlled trials. Most of the others are early, mixed or based on very small studies.
The strongest evidence sits with muscle soreness. A Cochrane review of 17 randomized trials covering 366 participants found that cold water immersion reduced delayed onset muscle soreness at 24, 48, 72 and 96 hours after exercise, compared with passive rest.
A 2025 network meta-analysis pinpointed the practical dose. Ten to fifteen minutes at 11 to 15 degrees Celsius (52 to 59 Fahrenheit) came out as the most effective protocol for reducing soreness after exercise-induced muscle damage.

What is well supported
The muscle recovery data comes from repeated small trials, all pointing the same way. Cold water immersion consistently blunts the ache of hard training in the days that follow. The Cochrane authors added that the studies were low quality overall, and that the optimum method and long-term safety are not yet clear.
The acute neurochemical response is also well documented. A Czech study of ten young men immersed to the neck at 14 degrees Celsius reported plasma noradrenaline rising to 530 percent of baseline and dopamine to 250 percent.
That rush helps explain the surge of focus and mood lift plungers describe stepping out of the tub, though the sample was small and the effect is short-lived.
What is early or mixed
A systematic review published in PLOS One in January 2025, pooling 11 randomized trials and 3,177 participants, gave the fullest picture yet. Stress dropped 12 hours after a cold plunge, but not immediately, and not at 24 or 48 hours.

Sleep quality was better in one included trial. Effects on mood were not significant in the eligible data.
Immune benefits are also unsettled. The same review found no immediate changes in immune markers, but referenced a workplace trial in which cold showerers logged a 29 percent reduction in sickness absence, a result that has not been replicated at scale.
Brown fat activation, another popular claim, is real in imaging studies at mild cold, but has not translated into the sustained metabolic transformation influencers describe.
What the evidence does not support
Detoxification is not a claim the trial literature supports. Neither is a lasting boost to resting metabolism from a couple of minutes in an ice tub. Post-workout cold immersion may even be counterproductive for strength athletes.

Harvard Health cardiologist Prashant Rao noted that "the little evidence we have suggests that post-exercise cold therapy may have detrimental effects on gains in muscle power and strength."
The heart risk people underestimate
The first seconds in cold water trigger a cold shock response, a sharp rise in heart rate, blood pressure and breathing that is well studied in drowning research. If an involuntary gasp happens while the head is submerged, the resulting lungful of water can be deadly, Smithsonian Magazine noted in its physiology explainer on polar plunges.
Harvard Health cautions that anyone with a heart rhythm disorder, prior cardiovascular disease, circulation problems, peripheral artery disease or Raynaud's syndrome should avoid cold plunges. The extra adrenaline can disturb heart rhythm, though this usually is not a problem in someone with a healthy heart.
Heather Massey, a cold water physiologist at the University of Portsmouth, told Smithsonian Magazine that entry should be gradual. "We suggest that you go in slowly and gradually rather than jump or dive in," she said, adding that core temperature keeps falling for 30 to 50 minutes after the person is out of the water.

Rao's bottom line at Harvard was blunter. "I'm pretty cautious about recommending cold-water therapy, especially because the reasons for doing it can be much better served with other interventions, such as exercise," he said.
If muscle soreness is the target, the trial evidence points to a narrow, specific window. Ten to fifteen minutes at 11 to 15 degrees Celsius after hard exercise is the range with the strongest support, and anyone with a diagnosed heart condition should clear it with their own doctor first.
