FDA approves the first weight-loss pill you can take any time, without food restrictions

A new daily GLP-1 pill drops the fasting window that has frustrated Rybelsus users.

On April 1, 2026, the FDA cleared Eli Lilly's Foundayo (orforglipron), a once-daily oral GLP-1 receptor agonist for adults with obesity, or overweight with at least one weight-related condition. The label carries no timing rule at all. The pill can be swallowed with or without food, with or without water, at any point in the day.

That is the practical shift. The other oral GLP-1 on the market, Rybelsus (oral semaglutide, Novo Nordisk), has to be taken first thing in the morning on an empty stomach, with no more than about four ounces of plain water, followed by a 30-minute wait before eating, drinking or taking anything else. Foundayo drops that window entirely.

What the phase 3 data actually showed

Orforglipron is a small-molecule, nonpeptide GLP-1 receptor agonist from Eli Lilly, meaning it activates the same appetite and blood-sugar pathway as injectable drugs like semaglutide and tirzepatide, but in a pill that does not need to be refrigerated or injected. In the phase 3 ATTAIN-1 trial of 3,127 adults with obesity or overweight with a weight-related condition, participants on the highest dose who stayed on treatment lost an average of 27.3 pounds, or 12.4% of body weight, over 72 weeks. The placebo group lost 2.2 pounds, or 0.9%.

The label offers six strengths, 0.8 mg, 2.5 mg, 5.5 mg, 9 mg, 14.5 mg and 17.2 mg, with patients titrating up over several months to manage tolerability. The most common side effects reported in the trials were gastrointestinal, including nausea, constipation, diarrhea, vomiting, indigestion and stomach pain. Headache, fatigue, belching, heartburn, gas and hair loss also appeared on the label. These are broadly in line with what has been reported for other GLP-1 medications, where injection-site issues are traded for stomach symptoms during dose escalation.

What it costs and where to get it

Lilly began accepting prescriptions through its direct-to-patient service, LillyDirect, the day of approval, with shipping starting April 6, 2026. Broader distribution through retail pharmacies and telehealth providers followed shortly after.

Pricing depends on coverage. Patients with commercial insurance may pay as little as $25 a month with a Lilly savings card, according to the company. Self-pay through LillyDirect starts at $149 a month at the lowest dose. For Medicare Part D beneficiaries, Lilly has said a $50 monthly out-of-pocket price will be available beginning July 1, 2026.

"As a convenient, once-daily oral pill that delivers meaningful weight loss, this is obesity care designed for the real world," David A. Ricks, chair and chief executive of Eli Lilly, said in the company's approval announcement.

Who it is meant for

Foundayo is approved for adults with a body mass index of 30 or higher, or 27 and above with a weight-related condition such as type 2 diabetes, high blood pressure or high cholesterol. It is intended to be used alongside a reduced-calorie diet and more physical activity, not on its own. Anyone considering an oral GLP-1 should discuss it with their doctor, particularly if they take other medications or have a history of thyroid or pancreatic disease.

For patients already on Rybelsus who have struggled with the morning fasting routine, Foundayo is the first FDA-cleared oral GLP-1 that lets them take a pill with breakfast, at lunch or before bed.
