For depression and anxiety, one type of exercise consistently outperforms the rest

Aerobic activity, meaning brisk walking, cycling, swimming or running, is the exercise type most consistently linked to bigger improvements in depression and anxiety. That is the practical takeaway from a 2025 meta-analysis of 32 randomized controlled trials with 3,243 participants, corroborated by a 2026 umbrella review covering more than 1,000 component studies.

The 2025 meta-analysis was published in the International Journal of Mental Health Nursing by a team led by Harry Banyard. Across the 25 trials pooled for depression, exercise produced a large effect on depressive symptoms (standardized mean difference of -0.97, 95% CI -1.28 to -0.66). Across the 11 trials pooled for anxiety, it produced a moderate effect (-0.66, 95% CI -1.09 to -0.23).

That is a striking size. Meta-analyses of new-generation antidepressants against placebo typically land at effect sizes of around 0.27 to 0.30 on the standard depression scales, well below what these exercise trials delivered.

Aerobic training led the pack

When the Banyard team split the depression trials by exercise type, the aerobic subgroup showed the biggest effect, with a standardized mean difference of -1.60 across 18 studies. Resistance or mixed programs came in at -0.89 across 6 studies. The confidence intervals overlap, so aerobic was not proven superior in a formal head-to-head test, but the point estimates clearly favored it.

A 2026 umbrella review in the British Journal of Sports Medicine, led by Neil Richard Munro, pooled 63 systematic reviews with 81 meta-analyses, 1,079 component studies and 79,551 participants. The authors concluded that all exercise formats helped mitigate depression and anxiety symptoms, with aerobic exercise having the most substantial impact on both conditions. Exercise overall reduced depression by an SMD of -0.61 and anxiety by -0.47 in that analysis.

What counts as aerobic

Aerobic activity is anything that raises the heart rate and keeps it there. Brisk walking, cycling, swimming, jogging and dancing all qualify. The Banyard trials used mostly walking and running.

World Health Organization guidelines, cited in the Banyard paper, recommend 150 to 300 minutes of moderate-intensity aerobic activity a week, or 75 to 150 minutes of vigorous activity, plus muscle-strengthening on two or more days. That is roughly a 30-minute brisk walk on most days of the week.

Any movement beats none

The Banyard authors flagged one detail that matters for people put off by the WHO numbers. Only three of the 30 trials included in their review actually met the full guideline dose for both aerobic and strengthening activity. Exercise still improved symptoms in the rest. Doing something is better than doing nothing.

Anyone with a diagnosis of depression or anxiety should talk to a doctor before starting a new exercise program or adjusting any current treatment. Exercise is best thought of as an addition to standard care, not a replacement.

The signal from the recent evidence is nonetheless clear. Movement helps, and when the goal is easing depression or anxiety, aerobic activity is the format with the strongest research to back it.
