A 2022 systematic review of 18 clinical trials, published in Critical Reviews in Food Science and Nutrition, found that sourdough bread caused a smaller post-meal rise in blood glucose than conventional industrial bread.
The effect was strongest when the loaf was made with whole wheat flour.
The mechanism is chemistry, not marketing. During long fermentation, lactic and acetic acid bacteria produce organic acids that lower the bread's pH and alter its starch structure.
That process can slow how quickly glucose enters the bloodstream after a meal.

A separate review in Advances in Nutrition analyzed 25 clinical trials covering 542 people. Sourdough fermentation lowered the share of rapidly digestible starch and raised resistant starch.
But the authors cautioned that results varied by grain type. The pattern was more consistent in whole-grain wheat than in white bread.
How much the spike softens
The Rolim review found sourdough reduced blood glucose at both 60 and 120 minutes after eating, compared to industrial bread. A 2024 study in Frontiers in Nutrition measured the glycemic index of three part-baked sourdough loaves.
The traditional white version came in at around 88. Wholemeal and cereal-based versions dropped to about 68. That gap tracks the wider review evidence. What is inside the flour matters as much as the fermentation.

Effects on hormones that govern appetite, including ghrelin, GLP-1 and GIP, were also lower after sourdough meals in some trials, but that finding rests on smaller samples.
The clinical evidence is thinner than the marketing
Sourdough is often sold on gut-health and metabolic claims. The clinical picture is more restrained.
A 2023 review in Frontiers in Nutrition, from researchers at BOKU University in Vienna, the University of Alberta and Maastricht University, concluded that significant health effects on well-defined clinical endpoints have not been clearly shown.
Neither the European Food Safety Authority nor the US Food and Drug Administration has approved a sourdough health claim.

The Advances in Nutrition authors wrote that whether sourdough fermentation on its own has a real impact on postprandial blood glucose is equivocal. Flour type and bread density likely contribute more to a bread's glycemic index than the leavening method.
Why most supermarket sourdough is not the real thing
Almost all of the clinical evidence rests on long, slow fermentations, usually 12 to 36 hours with wild lactic acid bacteria and yeast.
Most supermarket loaves labeled sourdough are made with added baker's yeast, sped-up fermentation and sometimes vinegar or other acids added to mimic the tang.
The 2023 D'Amico review flagged that most commercial products they surveyed had pH levels above 5.0, well above the pH below 4.5 typically needed for the fermentation-driven changes to take hold.

The label check is short. Look for flour, water, salt and a starter or live culture. If yeast, baker's yeast, vinegar, acetic acid, dough conditioners or preservatives appear, the shortcuts have been taken.
