Girls as young as 4 could now be offered a vaccine that most families do not think about until middle school.

Most parents know the HPV shot as something that comes up around middle school.

You hear about it at the preteen checkup, usually alongside a stack of other reminders. For many families, it feels like a decision for later.

That decision could now move much earlier. The World Health Organization's vaccine advisory group said on Tuesday that girls as young as 4 may be offered the HPV shot if it helps lift stubbornly low coverage rates.

The Strategic Advisory Group of Experts on Immunization, known as SAGE, still names girls aged 9 to 14 as the main target for a single-dose vaccine.

The 4 to 8 window is an option on top of that, not a replacement.

Why the age shift is on the table

The gap between where coverage sits and where WHO wants it is wide.

According to Insider Paper, SAGE chair Anthony Scott said global HPV vaccine coverage stands at 33 percent, far short of the 90 percent target.

That target sits inside WHO's wider plan to eliminate cervical cancer. According to the World Health Organization, there were around 604,000 new cervical cancer cases worldwide in 2024. About 280,000 women died from the disease that same year.

Most of that burden falls on low- and middle-income countries, where screening and vaccination are hardest to access.

SAGE's view is that giving countries flexibility on the starting age may help them close the coverage gap faster.

What it means for US families

In the US, the schedule has not moved to age 4.

The Centers for Disease Control and Prevention still recommends routine HPV vaccination at ages 11 or 12, with the option to start as early as age 9.

Catch-up shots are recommended through age 26 for anyone who missed the earlier window.

After 26, the CDC does not routinely recommend the vaccine, though adults up to 45 can discuss it with their doctor.

What parents can ask at the next checkup

The WHO announcement does not change the US schedule on its own.

Parents of young children can ask their pediatrician when the HPV shot is planned, and parents of older children can check whether a catch-up dose is still on the table before age 26.
