Pregnant while on Ozempic or Wegovy? Here is what the evidence says

A major new analysis of GLP-1 use around conception lands with one reassuring message and one number worth a closer look.

You may have heard the phrase "Ozempic baby."

Clinics report seeing more women who become pregnant unexpectedly while taking a weight-loss shot. That has left quiet worry about what such an exposure might mean for a baby in the earliest weeks.

A new review of the evidence brings largely reassuring news.

A systematic review and meta-analysis was published on September 29, 2026, in The Lancet Obstetrics, Gynaecology and Women's Health.

It found no increased risk of major birth defects, preterm birth, stillbirth or unusual birth size linked to GLP-1 exposure around conception.

The work was led by Professor Asma Khalil at City St George's, University of London. It was presented at the European Association for the Study of Diabetes annual meeting in Milan.

Reassurance on birth defects and preterm birth

The team pooled data from seven observational studies covering tens of thousands of women with type 1, type 2 or gestational diabetes who took a GLP-1 drug around conception.

These drugs include semaglutide, the active ingredient in Novo Nordisk's Ozempic and Wegovy.

They also include tirzepatide, the active ingredient in Eli Lilly's Mounjaro. Congenital anomalies, high blood pressure disorders, neonatal death and gestational diabetes all landed with a null result in the pooled data.

According to Khalil, the findings should help clinicians offer evidence-based advice to women who realize they are pregnant while still on one of these drugs.

She also stressed that the current guidance to stop the medication before trying to conceive still stands.

The one number that needs a closer look

The analysis flagged one signal. Early pregnancy loss, meaning pregnancies ending before 14 to 22 weeks, was 31 percent more common among women exposed to GLP-1 drugs. That figure comes from two studies covering about 41,000 women.

The caveat matters. In the data pooled for that finding, miscarriage and deliberate termination of pregnancy were counted as the same outcome.

About 99 percent of the pregnancies in that pool were classed that way, so there is no way to tell how many were miscarriages and how many were terminations.

Independent experts urged even more caution. Nerys Astbury, associate professor of diet and obesity at the University of Oxford, told the Science Media Centre that the confidence intervals for most of the findings crossed the line of no effect.

That means the data are compatible with no association, but also with some benefit or some harm.

What the current advice says

UK regulators advise stopping semaglutide at least two months before trying to conceive, and tirzepatide at least one month before.

In the US, the labels for Ozempic, Wegovy, Mounjaro and Zepbound tell women to stop the drug when a pregnancy is planned or confirmed.

Women who find themselves already pregnant on one of these drugs are advised to contact their doctor rather than wait for the next check-up.
