Researchers ran a two-year walnut trial: The most harmful LDL particles fell

Adding roughly a half cup of walnuts to the daily diet lowered a heart-damaging type of LDL cholesterol in a two-year randomized trial of 708 older adults. The number of small, dense LDL particles fell 6.1%, and total LDL cholesterol dropped by an average of 4.3 mg/dL.

The findings from the Walnuts and Healthy Aging trial were published in the American Heart Association journal Circulation in 2021. Participants, aged 63 to 79, were recruited in Barcelona, Spain and Loma Linda, California, and 90% of them completed the two-year protocol.

Total cholesterol fell by 8.5 mg/dL on average. Triglycerides and HDL cholesterol did not change, and daily walnut eaters did not gain weight compared with the control group.

Why the small LDL number matters more

Total LDL is only half the story. LDL particles come in different sizes, and the smaller, denser ones are considered the most atherogenic, the type most tightly linked to the plaque that builds up in artery walls and drives heart attacks and strokes.

Their small size helps them slip into the artery wall more easily, and they are more prone to the oxidation that fuels vessel inflammation. That mechanism is why a 6.1% cut in small LDL particles counts as clinically interesting even when the drop in total LDL is modest.

According to the researchers, walnuts appear to improve the quality of LDL particles, not just the overall amount. The senior author was Emilio Ros of the Lipid Clinic at Hospital Clínic of Barcelona.

Men saw a bigger effect than women

The walnut diet cut LDL cholesterol by 7.9% in men and 2.6% in women. The trial did not pin down a mechanism for the sex difference, but the interaction was statistically significant.

The baseline matters too. Participants were largely healthy, close to half were already treated for high blood pressure or high cholesterol, and 32% were on statins, so their cholesterol was mostly in a normal range. In people with elevated cholesterol at the start, the reduction from a nut-enriched diet may be larger.

Where walnuts fit in a heart-healthy diet

Walnuts are a concentrated source of the plant-based omega-3 fatty acid alpha-linolenic acid. The American Heart Association's 2021 dietary guidance points to nuts and legumes as preferred protein sources for cardiovascular health, alongside fish, whole grains and unsaturated plant oils.

A practical takeaway is that swapping typical snack foods like chips or crackers for a small handful of walnuts is a low-friction way to add unsaturated fats without gaining weight.

The trial was funded by the California Walnut Commission, an industry body, though it was designed as a randomized controlled trial with two independent centers. People with high cholesterol, established heart disease or on lipid-lowering medication should talk to their doctor before making significant changes to diet or treatment.
