A 2024 systematic review published in Frontiers in Physiology pooled 16 trials of blueberry consumption and found consistent improvements in how blood vessels behave. Flow-mediated dilation, a standard measure of endothelial function, rose by 1.50 percentage points, and diastolic blood pressure fell by 1.95 mmHg on average. Roughly 400 adults were included across the trials.
The effects are modest at the individual level but meaningful across a population. Small shifts in blood vessel function and diastolic pressure track with lower cardiovascular risk over time.
What the vascular data actually show
The pooled analysis was led by Bixin Deng and colleagues at Sichuan University. Their conclusion was that blueberry consumption can significantly improve endothelial function and diastolic blood pressure, based on 11 studies included in the meta-analysis portion.
The team also flagged a stronger response in one subgroup. In smokers, systolic blood pressure dropped by 3.92 mmHg on average, a larger effect than in the wider sample. That fits the wider picture that endothelial damage is more pronounced in smokers, leaving more room for improvement.

Dose mattered too. The analysis showed that flow-mediated dilation improvements grew as anthocyanin intake rose, with the strongest effect at higher anthocyanin levels.
Anthocyanins, nitric oxide, and blood vessels
The proposed mechanism is well mapped. Anthocyanins, the deep-blue pigments in blueberries, and their gut-derived metabolites appear to raise nitric oxide availability in the lining of blood vessels. Nitric oxide relaxes vascular smooth muscle, which improves dilation and eases blood pressure.
A separate 2024 review in Frontiers in Nutrition, Blueberry Health Benefits: State of the Science, described anthocyanins and their metabolites as key mediators of blueberries' vascular-protective effects. The same review noted that intake of more than one serving of blueberries per week has been associated with a 10 percent lower risk of hypertension in observational data.
The brain evidence is narrower but real
A separate 2025 meta-analysis in Biogerontology pooled nine randomized controlled trials with 513 older adults who had mild cognitive impairment or subjective cognitive decline. Chronic blueberry intake was linked to a small but statistically significant improvement in episodic memory, and to better language memory in the mild cognitive impairment group.
The picture is not uniform. The same analysis found no meaningful improvement in processing speed, recognition memory, visuospatial learning, or working memory. And in adults with metabolic syndrome but normal cognition, a six-month trial in the American Journal of Clinical Nutrition found no strong cognitive, mood, or alertness benefit from 1 cup of blueberries a day.
In practice, the brain signal looks strongest in older adults already showing early cognitive decline, and much weaker in healthy midlife adults. That distinction matters when the fruit is being sold as a brain food.
What a daily habit looks like
Trials that saw benefits typically used the equivalent of about half a cup to two cups of fresh blueberries a day, over 12 to 24 weeks. That is a realistic amount to add to breakfast oats, yogurt, or a smoothie without upending a normal diet, and frozen berries carry the same anthocyanins as fresh.
Blueberries are not a treatment for high blood pressure or memory loss, and readers already on blood pressure medication or with concerns about cognitive changes should raise those with their doctor rather than swap medicine for fruit. But as an everyday habit, the evidence for a daily handful has quietly grown stronger.
