A brain imaging study of 39,771 middle-aged adults found that people sleeping under seven hours a night, and those sleeping nine or more, had more of the small brain lesions doctors treat as early warnings of stroke and dementia.
The finding, from a Yale School of Medicine team led by postdoctoral fellow Santiago Clocchiatti-Tuozzo and senior author Guido Falcone, was published in the Journal of the American Heart Association and drew on brain MRI data from the UK Biobank.
The lesions are white matter hyperintensities, small bright spots on MRI scans that signal small vessel disease in the brain. They are usually silent, so people do not feel them.
Their presence and volume predict a higher risk of stroke and dementia years down the line. The Yale team also measured fractional anisotropy, a marker of the structural integrity of the brain's wiring.

Among the participants, about one in five reported short sleep and 6 percent reported long sleep. Both groups had worse brain health markers than those hitting the seven to nine hour range.
The link held even after the researchers adjusted for high blood pressure, diabetes and smoking, three of the biggest known contributors to small vessel disease.
Why sleep sits alongside blood pressure
The finding places sleep on a similar footing to more familiar cardiovascular risks. "Conditions like stroke or dementia are the end-stage result of a long process that ends tragically," Clocchiatti-Tuozzo told reporters, framing sleep as a lever people can pull decades before that endpoint.
The American Academy of Sleep Medicine and the Sleep Research Society recommend that adults sleep seven or more hours per night on a regular basis to promote optimal health. That target lines up with the American Heart Association's Life's Essential 8 checklist.

Why the same sleep affects brains differently
A separate 2026 study by researchers at Edith Cowan University in Australia may help explain why the same short night hurts some people more than others.
The team looked at variants of the AQP4 gene, which helps clear waste from the brain during sleep, and compared them with participants' sleep patterns and brain scans.
Some AQP4 variants were tied to faster gray matter loss in people who slept less. Others were associated with structural brain changes in people who took longer to fall asleep.
Lead author Dr. Ayeisha Milligan Armstrong said the interaction between genetics and daily habits matters as much as the genes themselves. The Edith Cowan team suggested future clinical trials that fold in genetic information, to test whether improving sleep can reduce inherited vulnerability to Alzheimer's-related brain changes.

In the meantime, the practical target has not changed. Aim for seven to nine hours a night, and if you regularly fall well outside that band, treat it as something worth raising with a doctor.
