Struggling to finish a sentence may say more about brain health than briefly forgetting a name. A February 2024 study in Alzheimer's and Dementia found that slower speech and longer between-utterance pauses during memory recall lined up with early tau protein buildup in the brain.
Tau is one of the first biological markers of Alzheimer's disease. Standard memory scores did not track the same signal.
The Stanford and Boston University team analyzed 238 cognitively unimpaired adults aged 32 to 75 from the Framingham Heart Study. Participants recalled a story they had been told 20 minutes earlier while researchers timed the pauses, counted them, and measured how fast the words came out.
Longer and more frequent between-utterance pauses, together with slower speech rate, were associated with higher tau signal in the medial temporal and early neocortical regions of the brain. The delayed recall score itself, the number typically used in a clinic, showed no such link.

Predicting the disease six years out
A separate Boston University team followed 166 people already living with mild cognitive impairment, a stage where memory or thinking is noticeably off but daily life still works. Ninety later progressed to Alzheimer's disease and 76 remained stable.
Their AI model, trained on transcripts of standard cognitive tests from the Framingham Heart Study, predicted who would progress with 78.5% accuracy and 81.1% sensitivity over six years in a study published in Alzheimer's and Dementia in June 2024.
"It shows the power of AI," senior author Ioannis Paschalidis of Boston University told PsyPost, saying the team could predict progression over the next six years "with relatively good confidence and accuracy." Co-author Rhoda Au framed why voice matters as a screening signal. "Digital is the new blood. You can collect it, analyze it for what is known today, store it, and reanalyze it for whatever new emerges tomorrow," she said.
When word-hunting stops being ordinary
Occasional word searches, calling a fork a "thing" or losing a name for a beat, are common at any age. The signal these studies measured is different. It is persistent slowing, longer gaps between utterances, and more frequent breaks that worsen over months.

The pattern is often clearer to family than to the person speaking. Because speech markers can shift before conventional memory tests turn abnormal, the Boston University authors described their approach as an inexpensive and broadly accessible screening tool that could work alongside other tests and biomarkers.
