Three-quarters of common sweeteners may be quietly changing your gut bacteria

A new laboratory study from Cambridge researchers tested 39 widely used sweeteners and found that around three-quarters of them altered the growth of at least one type of gut bacteria, with several slowing species tied to a healthy gut. The findings were published in Molecular Systems Biology in July 2026 by scientists at the Medical Research Council Toxicology Unit at the University of Cambridge.

The work was done outside the human body, in cultures of 25 bacterial species chosen to represent the gut, and the authors are clear that human studies are still needed before firm health conclusions can be drawn.

What the researchers actually tested

The team screened a mix of artificial sweeteners, natural sweeteners and sugar alcohols, then repeated the experiments with common companions that end up in the same meal or medicine cabinet, including caffeine, the food additives vanillin and advantame, and eight widely used medications.

More than 100 interactions turned up when a sweetener met another compound, according to the Cambridge team. In 34 cases the effect on bacteria got stronger, in 68 it got weaker. In other words, the response depended not just on the sweetener but on what it was mixed with.

The most striking result came from a stevia-derived compound called isosteviol paired with duloxetine, an antidepressant used by millions of US patients. Together they sharply suppressed Roseburia intestinalis and Parabacteroides merdae, two species linked to a healthy digestive system.

Sonja Blasche, the study's lead author at the MRC Toxicology Unit, said sweeteners are often marketed as metabolically neutral, but the study challenges that idea. Ashkan Farhadi, a gastroenterologist not involved in the work, described it to Medical News Today as one of the most comprehensive studies of its kind, while the International Sweeteners Association cautioned that lab dishes cannot replicate the full complexity of the human gut.

Why gut bacteria matter

The gut microbiome is not a passive passenger. Bacteria there produce short-chain fatty acids that help regulate energy metabolism, support the intestinal lining and shape immune responses, according to a 2026 review in Frontiers in Nutrition. Shifts in this ecosystem have been linked to obesity, inflammation and metabolic disease.

Sugar substitutes have been under wider scrutiny for similar reasons. A meta-analysis of 21 randomized trials, published in June 2026 in Current Atherosclerosis Reports, reported that non-nutritive sweeteners raised fasting insulin and long-term blood sugar markers in adults, and altered gut microbiota composition. Dariush Mozaffarian, cardiologist and director of the Food is Medicine Institute at Tufts, said use of these sweeteners has outpaced understanding of their long-term health effects.

What this means at the grocery store

For now, the Cambridge findings are a signal, not a verdict. Digestion, diet, genetics and each person's existing microbiome can blunt or amplify what happens in a test tube.

Still, sweeteners are easy to overlook. They appear well beyond obvious "diet" sodas, showing up in yogurts, protein bars, flavored waters, chewing gum, sauces and many medications. Reading the ingredient list is the most practical step. Names to look for include sucralose, aspartame, saccharin, acesulfame potassium, stevia and steviol glycosides, monk fruit extract, erythritol, xylitol, sorbitol and maltitol.

Anyone taking daily medication and consuming a lot of low-calorie sweetened products who has ongoing digestive symptoms should raise the question with their doctor, rather than adjusting prescriptions on their own.
