Long-term stress does more than wear you down. Cardiology, immunology and neuroscience research now converge on a simple message. When the pressure never lets up, the body pays.
In a 2021 scientific statement, the American Heart Association concluded that psychological health can raise or lower cardiovascular risk on a scale comparable to high blood pressure, diabetes and smoking. The document was the AHA's first broad statement on the mind-heart-body connection.
One of the largest cardiovascular signals comes from work stress. A Lancet meta-analysis led by Mika Kivimaki pooled individual data on 197,473 workers from 13 European cohorts. After a mean follow-up of 7.5 years, employees reporting high job strain had a 23 percent higher relative risk of coronary heart disease than colleagues without it (hazard ratio 1.23, 95% CI 1.10 to 1.37). That is a modest but consistent effect on top of the traditional risk factors.
Most of what stress does to the body runs through cortisol, the main stress hormone. When stress becomes constant, cortisol stops rising and falling with the day and instead runs high in the background. That pattern is linked to elevated blood pressure and a stickier clotting profile, both of which push cardiovascular risk up over years.
How it weakens your immune defenses
Short bursts of cortisol can sharpen immune responses. Sustained high levels do the opposite. A study published in the journal Immunology in January 2026 found that chronic cortisol exposure inhibits T cell function, the white blood cells that hunt infected and abnormal cells. Cortisol dampened T cell proliferation and cut secretion of the signaling molecules IFN-gamma and TNF-alpha in a dose-dependent way.
The effect fits a long-standing observation from wound-healing research. People under sustained psychological stress heal skin wounds more slowly, in part because cortisol blunts the early inflammatory response that clears debris and calls in repair cells.
What years of stress do to the brain
The brain is not spared either. A 2026 review in Frontiers in Aging Neuroscience notes that chronically elevated cortisol is linked to reduced hippocampal volume, especially in people with chronic stress and mood disorders. The hippocampus is the seahorse-shaped region that anchors memory formation.
Structural MRI studies have documented the pattern across multiple populations with elevated cortisol. Some of it appears reversible. Patients successfully treated for Cushing's syndrome, a condition of chronic cortisol excess, regain part of their hippocampal volume on follow-up scans.

What actually helps
The evidence for lifestyle interventions is strongest for three habits.
Regular aerobic exercise is the most robustly tested. A one-year randomized trial published in March 2026 in the Journal of Sport and Health Science assigned 130 adults to either 150 minutes a week of moderate-to-vigorous aerobic exercise or a health-information control group. The exercise group ended the year with significantly lower long-term cortisol (between-group difference minus 0.62, 95% CI minus 1.14 to minus 0.10, p=0.039).
Consistent sleep timing is second. Even a few nights of short sleep push next-day cortisol up, and irregular bed and wake times track with higher anxiety and depression scores.
Social connection is third. Loneliness has been tied to higher circulating cortisol and impaired glucocorticoid receptor sensitivity, and to higher rates of cardiovascular disease and mortality.
