Only 1 in 7 online blood pressure images shows the right technique – are you measuring correctly?

When researchers went looking for accurate blood pressure images online, they found something unsettling. Out of 1,106 stock photos of people taking a reading, just 14.3% got the technique right across the board.

The analysis, published in the American Heart Association journal Hypertension in September 2025, pulled images from 11 large stock sites including Getty, iStock, Shutterstock and Adobe Stock. Two reviewers scored each photo against the 2023 International Consensus guidelines for standardized clinic measurement. Clinical-setting images fared worst at 7.9% accurate. Home-monitoring photos scored better but still landed at only 25.3%.

That matters because nearly half of American adults live with high blood pressure, and millions manage the condition partly through home checks. A reading off by even a few points can push someone across a diagnostic line, triggering medication changes or hiding real risk.

"We expected that about 50% of images would be accurate, however, our findings were worse than expected," said lead author Alta Schutte, Ph.D., of the University of New South Wales Sydney and The George Institute for Global Health.

Arm position is the most damaging error

The costliest mistake sits in the arm. In a randomized crossover trial at Johns Hopkins Medicine published in JAMA Internal Medicine in October 2024, researchers put 133 adults through blood pressure measurements with the arm in three positions. The reference position rested the arm on a desk at heart level. The other two rested the hand in the lap or let the arm hang unsupported at the side.

Compared with the desk-supported position, resting the arm in the lap overestimated systolic blood pressure by 3.9 mmHg and diastolic by 4.0 mmHg. Letting the arm hang at the side inflated systolic readings by 6.5 mmHg and diastolic by 4.4 mmHg.

Sherry Liu, an epidemiology research coordinator at the Welch Center for Prevention, Epidemiology, and Clinical Research, spelled out what that means at the exam table. "If you are consistently measuring blood pressure with an unsupported arm, and that gives you an overestimated BP of 6.5 mmHg, that's a potential difference between a systolic BP of 123 and 130, or 133 and 140 -- which is considered stage 2 hypertension," she said.

The other errors adding up on your reading

The Hypertension analysis flagged four more technique failures that dominate online imagery.

An unsupported back was the single most common problem, present in 73% of images. In 54.8% of photos, the forearm was not resting on a firm surface at heart level. More than a third, 36.3%, showed the person with feet dangling rather than flat on the floor. And 11.5% placed the cuff over clothing rather than on bare skin.

Crossed legs are a separate concern. A randomized study in the Journal of Hypertension found that crossing the legs at the knee during measurement raised systolic pressure by roughly 8 mmHg in people with hypertension and about 3 mmHg in those without.

Cuff size is the fifth big variable. A cuff too small squeezes harder than the arm needs and inflates the reading. A cuff too large under-reads. Manufacturers include sizing charts, and the right cuff matches the circumference of the upper arm.

The AHA protocol, in plain terms

Getting a reliable number at home is straightforward once the rules are clear.

Sit quietly for five minutes before the first reading. Keep feet flat on the floor, legs uncrossed and back supported by the chair. Place the cuff on a bare upper arm, and make sure the size fits your arm circumference. Rest the arm on a table so the cuff sits at the level of the heart. Do not talk, read or scroll during the measurement.

Take two or three readings a minute apart and use the average. Home monitors that measure at the upper arm and have been independently validated give the most reliable numbers.

Schutte pointed out that the problem is bigger than one bad image. "More people are checking their blood pressure at home. But because of the inaccurate depictions online, even on reputable websites, it is very likely that people who look for information on the internet about blood pressure will see these images and may use the incorrect technique at home," she said.

If your home readings look higher than you expect, technique is the first thing worth checking before assuming the numbers reflect a real change. Bring your monitor to your next appointment and ask your doctor to watch how you take a reading. A small correction in posture may change the number, and the treatment, more than you think.
