For a healthy adult sitting at sea level, a pulse oximeter should read between 95 and 100 percent. That is the normal band clinicians use. Once the number slips below 90 percent, you have crossed the line into hypoxemia, and the meaning of the reading changes from reassuring to a reason to act.

The everyday things that fake a low reading
Nail polish, especially dark shades, artificial nails, cold fingertips, tobacco use, and poor peripheral circulation can all pull a reading downward without any change in the actual blood oxygen. Even the pulse oximeter itself has a built-in margin. FDA guidance for clinicians notes that a device reading of 90 percent "may represent an arterial blood saturation of 86 to 94 percent." That is a wide window on a small screen.
Before assuming a low number is real, warm the hand, remove polish, hold the finger still, and repeat the measurement after a minute. If the number climbs back into the normal band, the first reading was noise.

Conditions that genuinely pull the number down
When the low reading is real, several conditions are the usual explanations. Chronic obstructive pulmonary disease (COPD) and other advanced lung diseases can cause resting, exertional, and nocturnal hypoxemia. Sleep apnea repeatedly drops oxygen during the night. Heart failure and severe anemia reduce how well oxygen is delivered even when the lungs work. Acute infections such as pneumonia or COVID-19 can lower saturation quickly.

At 90 to 92 percent, contact your health care provider, especially if it is a change from your usual. At or below 88 percent, or any reading paired with confusion, severe shortness of breath, chest pain, or bluish lips or fingertips, call 911 or go to an emergency department.
