Serum creatinine is one of the most familiar lines on a routine blood panel. It looks simple, one number and one range. In a landmark NHANES III analysis of 18,723 Americans, mean values ranged from about 0.88 to 1.10 mg/dL in women and 1.00 to 1.29 mg/dL in men, rising with age in both sexes.
The same analysis cautioned that a single cutoff can be misleading. How much creatinine a body produces depends heavily on how much muscle it carries, so identical readings can mean very different things in different people.
Creatinine is a waste product from the normal breakdown of muscle tissue, and healthy kidneys filter it out into the urine. The National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) says the blood creatinine level is used to estimate the glomerular filtration rate, or eGFR. That figure, not creatinine on its own, is what actually grades kidney function.
What counts as normal
Most US labs list an adult reference range around 0.7 to 1.2 mg/dL for men and 0.5 to 1.0 mg/dL for women, according to Medical News Today.

The NHANES averages fell inside these bands. Across the US adult population, mean values worked out to 0.96 mg/dL for women and 1.16 mg/dL for men.
Why men run higher than women
The gap is about muscle, not kidney function. Men on average carry more skeletal muscle than women, and muscle turnover is what generates creatinine each day.

The NHANES authors also reported higher averages in non-Hispanic Black participants than in non-Hispanic White or Mexican-American participants, again a signal of body composition rather than disease.
Why the number shifts with age
Kidney filtration rate slowly declines through adult life, which can push creatinine up. But the pattern is muddied because muscle mass shrinks after roughly age 60.
An older adult can lose meaningful filtering capacity and still show a creatinine value inside the reference range, simply because they are producing less of it. The National Kidney Foundation lists being over 70 and unusual muscle mass among the factors that reduce the accuracy of any creatinine-based estimate.
Ask for the eGFR, not just the creatinine
US kidney panels typically report an eGFR calculated from the same blood draw. NIDDK's thresholds are straightforward. An eGFR of 60 or more is in the normal range, below 60 may indicate kidney disease, and 15 or less is called kidney failure.

Age changes the picture here too. The National Kidney Foundation's own age-adjusted averages fall from an eGFR of 116 at ages 20 to 29 down to 75 at age 70 and above.

If a lab report shows only creatinine, patients can ask their doctor whether the eGFR was also calculated. Anyone with reduced muscle mass, chronic illness, or symptoms such as unexplained fatigue, swelling, or changes in urination should discuss the result with a clinician rather than rely on the creatinine line alone.
