Common drug shows promise in early back pain research

Researchers have uncovered a promising new use for a medicine already prescribed to millions.

Millions of people live with spinal disc disease, yet medicine still offers few ways to slow its progression. New research suggests an established osteoporosis drug could eventually become part of a very different treatment strategy.

For decades, scientists have known that damaged spinal discs are one of the leading causes of chronic back pain.

What they have not had is a medicine capable of preventing those discs from gradually breaking down. Instead, treatment has largely focused on relieving symptoms, with surgery remaining the only long-term option for severe cases.

A new study by researchers at the University of Edinburgh and the University of Bristol, published in Communications Biology, takes a different approach.

Rather than testing a new painkiller, the team investigated the biological events that occur long before spinal tissue becomes permanently damaged. Their findings, also reported by ScienceDaily, point to existing medicines that could potentially be repurposed in the future.

Following the biology

Instead of beginning with patients, the researchers turned to zebrafish, an animal widely used to study human genetics because many biological processes are remarkably similar.

The team examined what happened when a collagen-related gene linked to spinal health was no longer functioning correctly.

They found that the spine first lost part of its supporting structure before abnormal mineral deposits gradually formed, causing tissues to become increasingly rigid and, in some cases, leading neighbouring vertebrae to fuse together.

Those observations prompted a closer look at the biological mechanisms involved. The researchers identified changes in pathways controlling phosphate balance, lipid metabolism and cellular growth, suggesting that disc degeneration may be driven by several interacting processes rather than a single defect.

Repurposing existing medicine

One of the most notable findings came when the scientists tested interventions already available. A bisphosphonate, a class of drugs routinely prescribed for osteoporosis, reduced the abnormal mineral accumulation seen in the zebrafish.

Altering lipid metabolism produced similar effects, highlighting more than one possible route for future drug development.

The researchers caution that these findings remain experimental and have not been tested in people.

Even so, the study shifts attention from managing the consequences of disc degeneration to understanding — and potentially interrupting — the biological processes that cause it in the first place.
