How many push-ups should you be able to do? What the number reveals about your heart

Middle-aged men who could crank out more than 40 push-ups in a single set had a 96 percent lower risk of a cardiovascular event over the following decade than those who managed fewer than 10. That was the headline finding of a study of 1,104 active male firefighters published in JAMA Network Open in February 2019.

The push-up test, in other words, measured something well beyond arm strength. It tracked closely with future heart attacks, strokes and other cardiovascular outcomes, and it did so at least as reliably as a submaximal treadmill test.

What the Harvard study actually found

The researchers, led by Justin Yang of the Harvard T.H. Chan School of Public Health, followed the firefighters for roughly a decade. During 8,601 person-years of follow-up, they recorded 37 cardiovascular events. Nearly all of those events, all but one, occurred in men who had completed 40 or fewer push-ups at baseline.

Risk was substantially lower across every category above the reference group. Compared with the group that could do 10 or fewer, men who managed 11 to 20 push-ups had about a 64 percent lower risk. The 21 to 30, 31 to 40, and greater than 40 categories all showed reductions of 75 percent or more, with the top group landing at that 96 percent figure.

"Our findings provide evidence that pushup capacity could be an easy, no-cost method to help assess cardiovascular disease risk in almost any setting," Yang told the Harvard Gazette.

Senior author Stefanos Kales added that the results underscore why clinicians should be checking basic fitness during routine visits.

Two caveats matter. The men were occupationally active and middle-aged, with a mean age just under 40. And the study included no women, so the numbers cannot be assumed to translate directly.

Where you stand for your age

Widely cited fitness benchmarks put the push-up test in context by age and sex. The reference tables grade a single continuous set on a scale from very poor to excellent.

For men, an "average" score in the 20s sits at 17 to 29 push-ups. In the 30s it drops to 13 to 24, in the 40s to 11 to 20, and in the 50s to 9 to 17. By ages 60 to 65 the average band is 6 to 16. Anything above roughly 47 reps in your 20s is considered excellent.

Women's standards, using the same full push-up position, run lower. Average is 9 to 13 in the 20s, 7 to 12 in the 30s, 5 to 9 in the 40s, and 4 to 8 in the 50s. A modified version from the knees is common in home testing and produces higher counts across every age group.

How to test yourself

The protocol used in the Harvard study was simple. Start in a standard push-up position, lower until your chest is close to the floor, and continue at a metronome pace of 80 beats per minute until you cannot keep up or stop to rest.

At home, without the metronome, a continuous set to failure gives a reasonable estimate. Warm up first, keep your body in a straight line and count only reps with full range of motion.

A low number is not a diagnosis. Push-up capacity reflects muscular endurance, but it also picks up habits that shape heart risk, from consistent exercise to body composition. If your count is well below the average for your age, or if you have chest pain, breathlessness or a family history of early heart disease, that is a reason to talk with your doctor rather than to push through a harder workout on your own.

The upside is that the number can move. Training push-ups two or three times a week, alongside regular aerobic activity, typically produces meaningful gains within weeks.
