Turmeric sits on shelves next to fish oil and multivitamins, marketed for everything from arthritis to Alzheimer's to cancer. The bright yellow spice was one of the top-selling herbal supplements in US mainstream retail in 2024, with sales of $141.8 million.
When researchers actually look at the clinical trials, the picture is narrower than the marketing suggests. Two conditions have decent human evidence behind curcumin, the active compound in turmeric.
Joint pain is the strongest case. A randomized study of 139 people with knee osteoarthritis compared curcumin (500 mg three times daily) with the anti-inflammatory drug diclofenac (50 mg twice daily) over 28 days.
Both groups saw a similar drop in pain scores. Only 13 percent of the curcumin group reported adverse effects, against 38 percent on diclofenac. Zero curcumin patients needed acid-blocking medication, compared with 28 percent on diclofenac.

For upper-gut symptoms, a 2023 randomized trial in BMJ Evidence-Based Medicine enrolled 206 adults with functional dyspepsia (recurring stomach pain, bloating or early fullness without a clear cause). They were split between curcumin, the acid-reducing drug omeprazole (20 mg daily), or both.
After 28 and 56 days, all three groups had comparable symptom relief. Harvard Health noted that curcumin appeared as effective as omeprazole and was safe and well tolerated in the trial.
A preliminary cognitive signal
A 2024 systematic review in Cureus pooled curcumin trials in adults with and without neurocognitive disorders. It found that curcumin supplementation was linked to statistically significant improvements in cognitive scores, most consistently in working memory.
The reviewers cautioned that dosages, formulations and populations varied widely. Effects depended on using bioavailable preparations, and larger trials with standardized methods are still needed.

Where absorption breaks the promise
There is a catch. Standard turmeric powder is barely absorbed. A comprehensive 2022 review in Biomedicines reported that peak plasma curcumin after a 3.6-gram oral dose reached only 11.1 nanomoles per liter.
That is a fraction of what cell studies use to demonstrate biological effects. Curcumin is nearly insoluble in water, poorly absorbed in the small intestine and cleared rapidly by the liver.
Combining curcumin with piperine (a compound in black pepper) or with fats can raise absorption sharply, and most trial-grade supplements use enhanced formulations for that reason. A pinch of pepper on a curry does not turn ordinary turmeric powder into a therapeutic dose.
An unregulated shelf of pills
Turmeric supplements are sold as dietary supplements, not drugs, so the FDA does not test their content before they reach stores. Independent testing has found products with less curcumin than the label claims, and, occasionally, contamination with lead.

Cancer claims outrun the human data
Turmeric's reputation as a cancer fighter is where the gap between marketing and evidence is widest. The same 2022 Biomedicines review noted that of 288 curcumin trials registered on ClinicalTrials.gov, only 20 had published results, and among the 17 cancer studies, none had shown definitive efficacy in humans.
Most cancer findings come from cell cultures and animal experiments, which do not automatically carry over to people. Turmeric belongs in an honest middle ground. It looks useful for a narrow set of complaints and oversold for almost everything else.
