The US job with the highest share of radiation cancer deaths sits 35,000 feet up

Cabin crew and pilots die of radiation-related cancers at a higher rate than any other US occupation, including nuclear technologists who work directly with radioactive materials. That is the surprising takeaway from a new analysis published in JAMA Internal Medicine on August 17, 2026.

Researchers from Harvard Medical School and Brigham and Women's Hospital examined 12.7 million US death records from 2020 to 2024, covering 503 occupations.

Among flight attendants, 6.9 percent of deaths involved a radiation-linked cancer. Among pilots, the share was 6.7 percent. Nuclear technologists, used as a comparison group, ranked twelfth.

The pattern did not hold for cancers unrelated to radiation, where aircrew mortality sat near the median. That specificity is what makes the finding striking to the study team.

Why cosmic rays reach the cabin

Cosmic radiation is a stream of high-energy particles that constantly bombards Earth from the sun and from deep space. Most of it is absorbed by the atmosphere before it reaches the ground. At cruising altitude, roughly 35,000 feet, that shielding thins out considerably, so the dose a person receives per hour in a jet is far higher than at sea level.

The US Centers for Disease Control and Prevention classifies aircrew as occupationally exposed to cosmic ionizing radiation and estimates they can accumulate 3 to 6 millisieverts a year across a full career, decade after decade.

For comparison, a frequent flier taking 10 cross-country round trips in a year picks up roughly 0.4 millisieverts, according to the authors of the new study.

The trip length matters. A single long-haul flight from Athens to New York can deliver around 67 microsieverts, while some shorter flights come in below one microsievert.

Nuclear workers are monitored, aircrew often are not

The comparison with nuclear technologists is the part of the study most likely to catch readers off guard. People who handle radioactive isotopes for medical imaging and cancer treatment are subject to strict federal dose limits, badge monitoring, and shielding rules.

Commercial aircrew, despite receiving the largest mean annual radiation dose of any occupational group tracked by US authorities, are not covered by the same monitoring system.

Female flight attendants are also known to have higher rates of breast cancer and melanoma than the general population, patterns the National Institute for Occupational Safety and Health has tied to cumulative cosmic radiation exposure and to circadian disruption from crossing time zones.

The new study cannot prove causation on its own. The researchers did not have individual flight histories, career length, or route data, so they could not link exposure directly to death. What they could show is that the excess mortality is concentrated in the specific cancers biology would predict from radiation, and not in others.

What it means for passengers

The bigger question for most readers is whether ordinary flying poses a similar threat. The study authors were clear that it does not. The occupational finding sits on decades of daily exposure at altitude, not on a summer vacation.

Anyone with concerns about a personal cancer risk, especially frequent business travelers or crew members with a family history, should raise it with their doctor rather than draw conclusions from a single study.

The researchers argue that the more important action lies with regulators. Extending the dose monitoring that already applies to nuclear workers to the pilots and cabin crew logging thousands of hours in the stratosphere would be a natural next step.
