Daily mindfulness, gratitude and optimism practice moved cardiovascular markers in weeks, researchers report.

Structured psychological wellness programs can nudge blood pressure and inflammation downward in as little as eight weeks, according to a systematic review published this year in the journal Cardiology Clinics.

The pooled evidence covers 18 randomized controlled trials, most of them enrolling adults with elevated cardiovascular risk.

Lead author Rosalba Hernandez, a professor of social work at the University of Illinois Urbana-Champaign, and colleagues examined trials of mindfulness, gratitude journaling, optimism training and related exercises.

The strongest results appeared when participants practiced briefly each day and joined a weekly session for reinforcement, the University of Illinois said in its announcement of the paper.

What actually changed in the body

Across the trials, mindfulness-based programs delivered over eight weeks reduced systolic blood pressure and lowered inflammatory markers, including high-sensitivity C-reactive protein and fibrinogen, in people with high blood pressure or recovering from acute coronary events.

The size of the drop varied by intervention. A 12-week spirituality-based digital program lowered systolic blood pressure by 7.6 mm Hg and central systolic pressure by 4.1 mm Hg.

An eight-week program delivered through WhatsApp, pairing weekly coaching with brief daily tasks, produced the strongest changes in health behaviors such as diet and step count.

What the programs looked like

Most trials ran 6 to 12 weeks with weekly meetings and short daily activities at home.

Delivery ranged from telephone sessions and in-person groups to smartphone apps, text messages and hybrid formats mixing online and face-to-face contact.

Individual studies typically included 50 to 200 adults in their late fifties to mid-sixties, and women made up 35 to 55 percent of participants.

Some improvements appeared to travel through lifestyle change.

Motivational interviewing paired with mindfulness helped cardiac patients walk about 1,800 more steps a day, and several programs improved eating habits and medication adherence alongside the physical markers.

Earlier heart research

The pattern echoes a trial reported by the American Heart Association in November 2022, which tested an eight-week Mindfulness-Based Blood Pressure Reduction program in more than 200 adults with elevated blood pressure.

Six months in, systolic pressure had fallen by 5.9 mm Hg in the mindfulness group and by 1.4 mm Hg in the usual-care group. Participants in the mindfulness arm also reported eating more heart-healthy food and sitting less.

Alyssa M. Vela, a professor of surgery and of psychiatry and behavioral sciences at Northwestern University Feinberg School of Medicine and a co-author of the new review, said the pattern reinforces a broader case for treating psychological well-being as part of heart care.

The review's authors caution that most of the pooled trials were small and short, and that keeping the benefits likely depends on continuing the practice.
