Fish oil capsules crowd every supermarket shelf, but the evidence on what they actually do for the aging brain has always been messy.
A large 2025 meta-analysis now offers the clearest picture yet. The answer is modest, uneven benefits that depend heavily on who takes them.
Researchers pooled 58 randomized controlled trials involving thousands of adults in a systematic review published in August 2025 in Scientific Reports. Omega-3 supplementation produced significant improvements in perceptual speed, visuospatial function, primary memory and global cognition.
Attention and executive function, by contrast, showed no significant overall effect. The authors call the overall gain a modest improvement in cognitive function among adults and note that many of the included trials were of low methodological quality.

Dose mattered, but not linearly. Benefits clustered in the 1,000 to 2,500 milligrams per day range, and some cognitive domains peaked and then declined at higher intakes. More was not simply better.
Who is most likely to benefit
Most omega-3 research focuses on two long-chain fatty acids, EPA and DHA. Both are found mainly in cold-water fish such as salmon, mackerel and tuna.
DHA is the brain-relevant one. It is a major structural fatty acid in neuronal membranes and concentrates in the synapses where brain cells communicate.
That biology helps explain why some populations gain more than others from a supplement.

A 2022 review of 33 studies cited by the National Center for Complementary and Integrative Health suggests that dietary and supplemental omega-3s may protect against cognitive decline in healthy adults who do not yet have dementia.
People with low dietary DHA, meaning those who rarely eat fatty fish, and older adults with mild cognitive impairment appear most consistently in the trials that report benefit.
Where the evidence stops
Once Alzheimer's disease has been diagnosed, the picture flips. NCCIH says multiple high-quality reviews have found no convincing evidence that omega-3 supplements treat mild-to-moderate Alzheimer's.
The story on brain shrinkage is similarly split. A 2016 trial in 240 older adults with mild cognitive impairment reported that a year on 2 grams of DHA daily helped preserve hippocampal volume compared with placebo.

A 2026 trial in eBioMedicine tested the same 2-gram daily dose for two years in 365 older adults with low fish intake and elevated dementia risk.
The supplement reached the brain, raising cerebrospinal fluid DHA by around 17 percent, but it did not preserve hippocampal volume or improve cognition.
Food or a pill
For most people, food remains the simpler route. The strongest positive findings on cognitive decline are tied to eating fish.
A couple of fatty-fish meals a week supplies EPA and DHA together with protein, iodine and other nutrients a capsule cannot copy.

Supplements make more sense for people who dislike fish, follow plant-based diets, or have been advised to raise their intake. Anyone considering higher doses, especially those on blood-thinning medication, should talk to a doctor first.
