Garlic’s blood-pressure benefit is small on average, but bigger for obese adults and those already in the danger zone

A 2025 meta-analysis of 10 randomized controlled trials found that daily garlic supplementation lowered systolic blood pressure by an average of 4.21 mmHg, with a 95 percent confidence interval from -5.74 to -2.69 mmHg. Diastolic pressure fell by 3.13 mmHg.

The pooled analysis, available via PubMed Central, drew on 541 participants for the systolic outcome. The authors describe the overall effect on hypertension as limited, but note the benefit was clearly larger in specific groups, including people with obesity, adults aged 50 to 60, and those with a baseline diastolic pressure between 90 and 100 mmHg.

A 4 mmHg drop is modest but not trivial. For context, current AHA and ACC guidelines classify any reading of 130/80 mmHg or higher as hypertension, and lifestyle measures like the DASH diet, weight loss, and reduced sodium sit at the foundation of treatment before medication is added.

The active compound is S-allyl-cysteine

The reviewers looked closely at dose. A daily intake of 2.4 mg of S-allyl-cysteine, or SAC, produced the largest reduction in systolic pressure at 5.07 mmHg. SAC is a stable sulfur-containing compound present in aged garlic extract, a supplement form in which fresh garlic is aged over months to convert volatile compounds such as allicin into more consistent, standardizable molecules.

An earlier randomized controlled trial in Australia using Kyolic aged garlic extract reported a similar mechanism story.

The researchers describe SAC as the main vaso-active component associated with garlic's blood-pressure-lowering effect, and point to nitric oxide and hydrogen sulfide signaling as the pathways involved.

Culinary garlic contains SAC too, but at much lower and more variable levels than a standardized aged extract. That is one reason clinical studies tend to use capsules rather than raw cloves when they want a reliable dose.

Timing and who benefits most

Trials that ran for 8 weeks showed the biggest drop, with systolic pressure falling by 5.69 mmHg.

Shorter interventions had smaller effects. Subgroup analyses also pointed to stronger responses in participants with a BMI in the obese range and in those whose diastolic reading sat between 90 and 100 mmHg at the start.

The authors caution that the number of high-quality trials remains small and that most interventions lasted only 8 to 12 weeks, so long-term effects are still unclear.

What to know before adding garlic

Side effects in the pooled trials were mild. Gastrointestinal discomfort and bad breath were the most commonly reported issues, and the overall adverse-reaction rate was slightly higher than placebo but not statistically significant. Garlic supplements can interact with blood thinners such as warfarin, so anyone on anticoagulants or existing blood pressure medication should talk with a doctor before starting.

The bottom line from the meta-analysis is that garlic is a plausible add-on for people looking to nudge blood pressure down through diet, especially those with elevated baseline readings, but it is not a substitute for the medical treatment prescribed for hypertension.
