If you wear an Apple Watch, Garmin, Fitbit, or Oura Ring, you have probably seen a small number labeled HRV. It sits next to your resting heart rate and moves around from night to night. Most people never learn what it means.
HRV stands for heart rate variability, the millisecond gap between one heartbeat and the next. A healthy heart does not tick like a metronome. The gaps stretch and shrink from beat to beat as your autonomic nervous system, the part of the body that runs breathing, digestion, and stress response without conscious effort, keeps adjusting.
More variability generally signals a nervous system that shifts flexibly between the calming parasympathetic branch and the alert sympathetic branch. Lower variability tends to appear during stress, illness, poor sleep, or fatigue.
Why HRV adds something resting heart rate does not
Resting heart rate tells you how fast the pump is running. HRV tells you how well the control system is responding. Two people can share the same resting pulse of 60 beats per minute while sitting on very different levels of autonomic reserve.

Wearable makers picked up on this because HRV correlates with an underlying biological state that matters. A 2026 review in Nutrients notes that the RMSSD version of HRV "serves as an indirect inflammation indicator, with published studies showing inverse correlations with inflammatory markers."
What "normal" HRV looks like
There is no single healthy number. HRV is highly individual and it declines with age as parasympathetic tone weakens. Fit adults in their twenties commonly sit far above adults in their sixties, and women and men can differ at the same age.
How well your wrist device actually captures the number is another question. A 2024 validation study of the Apple Watch Series 9 and Ultra 2 published in Sensors compared 316 measurements against a Polar H10 chest strap in healthy adults. Resting heart rate matched the reference to within 0.08 bpm, but HRV was underestimated by an average of 8.31 milliseconds with a mean absolute percentage error close to 29 percent. The authors concluded the watch's absolute HRV values should not be treated as equivalent to a medical-grade reading, though tracking the trend over time can still be useful.
Why cardiologists take the number seriously
The link between low HRV and worse outcomes is one of the more consistent findings in cardiovascular research. A meta-analysis in Europace pooled eight cohort studies covering 21,988 people without known heart disease. The lowest SDNN group carried a relative risk of 1.35 for fatal or non-fatal cardiovascular events compared with the highest.

A larger systematic review published in Neuroscience & Biobehavioral Reviews analyzed 38,008 participants across 32 studies. It found that "lower HRV parameter values were significant predictors of higher mortality across different ages, sex, continents, populations and recording lengths." The lowest quartile of a short RMSSD reading carried a hazard ratio of 1.56 for all-cause death.
These are association findings, not proof that raising your HRV will extend your life. But they explain why the metric has moved out of research labs and onto consumer wrists.
Four evidence-backed ways to raise it
Aerobic exercise is the most consistent lever. The American Heart Association's 2023 Update scientific statement on resistance exercise training, published in Circulation, confirms that regular training, including resistance work alongside cardio, produces favorable effects on cardiovascular health in adults with and without heart disease.
Sleep is the second. Deep sleep is when parasympathetic activity peaks and HRV climbs, which is why most wearables measure the number overnight. Chronic short sleep flattens that recovery window.

Alcohol is the fastest way to see your number drop. Controlled studies show that even moderate drinking suppresses HRV for hours, and the effect can carry into the following day.
Slow breathing is the fourth. A practical guide published in Frontiers in Public Health describes how paced breathing at roughly six breaths per minute, the resonance frequency for most adults, stimulates the baroreflex and boosts HRV during and after the session.
Watch the trend, not the day
A single-day HRV reading is noisy. Late meals, one drink, an argument, a warm bedroom, or menstrual cycle phase can swing the number by double digits with no lasting meaning.
The signal lives in the weekly and monthly trend line. If your seven-day average is drifting down while your training and stress are steady, something is worth attention. If it is climbing while you sleep more and drink less, the levers are working.

Anyone with symptoms of a heart rhythm problem, unexplained fatigue, or a sharply falling HRV trend they cannot explain should talk to a doctor rather than to a wearable app.
