Scientists mapped 830,000 immune cells in the Alzheimer’s brain and found something unexpected

A new study from the Icahn School of Medicine at Mount Sinai suggests the brain's own immune cells do not simply fail during Alzheimer's disease. A specific protective subtype actually grows more common as the disease advances, and researchers now believe it could become a target for future treatments.

The work, published in Nature Genetics on August 11, 2026, mapped more than 830,000 immune cells taken from the prefrontal cortex of 1,607 donors across the human lifespan. It is described by the research team as the most detailed map yet of how these cells shift with age and Alzheimer's pathology.

What microglia are, and why they matter

Microglia are the resident immune cells of the brain. They patrol brain tissue, prune connections between neurons, and clear debris, including the amyloid protein that builds up in Alzheimer's disease. Reviews of the field describe them as playing a dual role, protective in the early stages of disease and potentially harmful later on when chronic activation drives inflammation.

The Mount Sinai team sorted the cells into six subclasses containing 13 distinct subtypes, according to a report on the study. One of those subtypes, a disease-associated microglia state, became increasingly abundant as Alzheimer's advanced. Rather than accelerating damage, it appeared to enhance the clearance of harmful material from the brain.

A different target than amyloid

For the last several years, drug development has focused on stripping amyloid plaques out of the brain. Lecanemab, sold as Leqembi, is described by the Alzheimer's Association as an antibody intravenous (IV) infusion therapy that targets and removes beta-amyloid from the brain and modestly slows decline in people with early disease. It is not a cure, and it does not restore lost memory.

The Mount Sinai findings point in a different direction. Instead of stripping the plaque, the idea would be to reinforce the immune cells the brain is already recruiting against it. The researchers identified a molecular pathway involving three proteins, TREM2, MITF, and GPNMB, that appears to keep the protective microglial state switched on. Experiments in human tissue and in mouse models indicated that TREM2 signaling was essential for that benefit.

"We have uncovered potential new targets for therapies aimed at slowing Alzheimer's disease progression," said Donghoon Lee, assistant professor of genetics and genomic sciences and psychiatry at the Icahn School of Medicine at Mount Sinai and the study's first and corresponding author.

What it means for patients, and when

The scale of the problem is why any new lead attracts attention. The Alzheimer's Association estimates that more than 7 million Americans are living with Alzheimer's in 2026, a number projected to reach nearly 13 million by 2050 without a medical breakthrough.

Readers should treat this as an early lead, not a therapy that will be available soon. The paper is a large map of human brain tissue, backed up by animal work, that identifies a plausible target. Drugs designed to strengthen the TREM2 pathway are being explored in the wider field, but any medication that emerges from this specific finding would still need to move through years of clinical trials before doctors could prescribe it. Anyone worried about memory changes in themselves or a family member should talk to their physician rather than wait on future science.

The value of the new map is that it changes the picture. The brain, in this account, is not a passive victim of Alzheimer's. It is fighting back, and researchers now have a much clearer view of the cells doing the fighting.
